Özet

Miringoplasti yapılan kulakların retrospektif analizi yapılarak, -anatomik ve fonksiyonel kazanç irdelendi ve çeşitli prognostik faktörlerle sonuçlar arasındaki ilişki incelendi. 1987-1996 yıllan arasında, 6-60 yaş arasında (ortalama 22 11 yaş) 196 hastanın 225 kulağına miringoplasti yapıldı. Greft olarak 214 kulakta temporal kasfasyası, 11 kulakta tragalperi-kondrium kullanıldı. Underlay miringoplasti tekniği kullanılmıştır. Anatomik başarı, postoperatif ilk üç ay içinde greft in-takt ise erken başarı, birinci yılın sonunda greft intakt ise geç başarı olarak ifade edildi. Fonksiyonel kazanç, postoperatif birinci yıl sonunda hava-kemik eşikleri arasındaki aralığın desibel olarak belirtilmesinle ifade edildi. İstatistiksel değerlen¬dirmede ki-kare testi ye Fisher'in kesin testi kullanıldı. Ortalama takip süresi 12.3 6 aydır. Geç başarı değerlendirmesi, 83 kulağın yetersiz takibi nedeniyle, kulakların % 63.1 'inde yapılabildi. Erken başarı kulakların % 91.6'sında, geç başarı ise % 85.9'unda elde edildi. Perforasyon büyüklüğü, mastoid havalanma, ameliyat edilen kulağın durumu, östaki tüpü fonksiyonu ve hastanın yaşı gibi prognostik faktörlerle sonuçlar arasında istatistiksel olarak anlamlı bir ilişki bulunmadı (p>0.05). On desibel yada daha fazla işitme kazancı, kulakların % 66.4'ünde sağlandı. Kulak başına düşen ortalama işitme kazancı 16 desibel oldu ve kulakların % 91.8'inde hava-kemik aralığı 0-20 desibel arasına geldi. Miringoplasti ile çocuklarda ve eriş¬kinlerde oldukça iyi anatomik ve fonksiyonel sounçlar alınabilmektedir. Östaki tüp fonksiyonları bozuk olan kulaklarda pos¬toperatif rekraksiyon ve greft atwfisi bakımından dikkatli takip gereği vardır.

Abstract

A retrospective analysis has been made on ears which underwent myringoplasty in order to examine anatomi¬cal and functional succes, and to investigate the relation between the results and some prognostic factors. Between 1987 and 1996, 225 ears of 196 patients whose average age was 22 11 years (range 6 to 60 y eras) were operated. Temporalis fascia and tragal perichondrium were used as the graft material in 214 and 11 ears respectively. Underlay myringoplasty technique was used. Early surgical success was defined as an intact graft at least 3 months postoperatively and late surgical success was defined as an intact graft at the end of the first year postoperatively. Functional success was defined as the difference between air and bone thresholds in decibels at the end of the postoperative first year. Chisquare and Fisher's exact tests were used in statistical analyses. The average follow-up period was 12.3 6 months. Late surgical success could be evaluated in 63.1 % of the ears because of insufficient followup of 83 patients. Early surgical success in 91.6 % of the ears and late surgi¬cal success in 85.9 % of the ears were obtained. No statistically significant relationship was found between the prognostic factors such as the size of perforation, mastoid pneumatization, the conditions of the operated ears, ears, eustachian tube function and age of patient, and the results (p>0.05). Hearing gain often decibels or higher was obtained in 66.4 % of the ears. The average hearing gain was 16 decibels per ear, and in 91.8 % of the ears the air-bone gap closed to within 20 deci¬bels. Successful anatomical and functional results can be achieved by myringoplasty both ih children and adults. Close fol¬low-up is necessary from the viewpoint of postoperative retraction and graft atrophy for the ears having eustachian tube dis¬function.
