Seyrek rastlanan neoplazm tipleri olarak melanomlar en sık deride oluşur Melanomların tedavisi son 20 yılda önemli bir değişiklik göstermiştir ve elektif boyun diseksivonu tartışmaları halen devaın etmektedir. Melanomlu olgularda ilk olarak regional lenf nodu metastası araştırmaları yapılmalıdır Boynun palpasyonu lenf nodu araştırılması için önemli olmakla birlikte, metastas araştırılması için kaba bir yöntemdir Malign melanomlu bazı hastalarda palpabl adenopati bulunmadığı halde, bölgesel metastatik hastalık olabilmektedir. Primer alanla lenfatik bağlantı gösteren ilk nodlar sentinel lenf nodu olarak adlandırılır ve metastatik hastalığın ilk durağı olarak kabul edilir. Gama kameralar yardımıyla sentinel lenf nodu biyopsileri yapılarak, bulgular daha sonraki tedavinin planlarımasında kullanılabilir. Kliniğimizde 1994 ile 1998 yılları arasında toplam 241 olgu cilt neoplasmları nedeniyle tedavi edildi ve bunlardan üçünde malign melanom vardı. Elektif boyun diseksiyonu, bu üç olgunun sunulmasıyla birlikte tartışıldı.
Abstract
Melanomas are infrequent neoplasms and often appear on skin. Melanoma care has not changed significantly in the last 20 years, and the controversy of elective lymph node dissections in this disease continues to be discussed. The initial evaluation of melanoma patients should include an assessment of the regional lymph nodes for metastatic disease. Clinical palpation of the nodes is an important but relatively crude index of metas-tatic melanoma. Some patients presenting with malignant melanoma without adenopathy have regional metasta-tic disease. The concept of the sentinel lymph node as the first node in the lymphatic basin that drains the primary .site, has been documented to contain the first site of metastatic disease. A technique of gamma probedirected sentinel lymph node biopsy can be applied and the results can be used to direct further therapy. Between 1994 and 1998, a total of 241 patients were treated at our clinic for cutaneous neoplasms and 3 of them were malign melanoma of the head and neck. The role of the elective neck dissection was discussed with the pre-sentation of these 3 patients.
