Özet

Bu çalışmada, tonsillektomi spesmenlerinin rutin histopatolojik incelemesinin gerekliliği literatür bilgileri eşliğinde tartışılmıştır. Gereç ve Yöntemler: Çalışmamızda, Temmuz 2008-Ocak 2017 tarihleri arasında Fırat Üniversitesi Hastanesi Kulak Burun Boğaz Kliniği’ndetonsillektomi ve/veya adenotonsillektomi yapılan 1.152 hastanın dosyası yaş, cinsiyet ve histopatolojik sonuçları bakımından incelendi. Bulgular: Çalışmamızda tonsillektomi ve/veya adenotonsillektomi yapılan 1.152 hastanın 18’i malignite şüphesi ile opere edilmiş idi. Geriye kalan 1.134hastada beklenmedik malignite ile karşılaşılmadı. Sonuç: Tonsillektomi spesmenlerinde beklenmedik malignite oranının oldukça düşük olması nedeni ile histopatolojik incelemenin rutin olarakyapılması yerine, preoperatif muayenenin daha dikkatli yapılması gerekmektedir. Fizik muayenedetonsiller asimetri, sertlik, tonsil mukozasında ülserasyon veya boyunda palpabl lenf nodu bulgularından en az biri olan hastalarda, spesmenlerin histopatolojik incelenmesinin daha doğru olduğu düşünülmektedir.

Abstract

In this study, the necessity of routine histopathologic examination of tonsillectomy specimens was discussed in the context of literature. Material and Methods: In our study, the data of 1.152 patients who underwent tonsillectomy and/or adenotonsillectomy at Fırat University Department of Otorhinolaryngology between July 2008-January 2017 were reviewed for age, sex and histopathologic results. Results: Only 18 of 1.152 patients underwent tonsillectomy and/or adenotonsillectomy with the possibility of malignancy. Unexpected malignancy was not encountered in the remaining 1.134 patients. Conclusion: Because the rate of unexpected malignancy is very low in tonsillectomy specimens, it is necessary to perform preoperative examination more carefully than histopathologic examination routinely. We think that the histopathological examination of specimens is more accurate in patients with at least one of the symptoms of tonsil asymmetry, hardness, ulceration of the tonsil mucosa or palpable lymph node in the physical examination.
