Özet
Konjenital kulak atrezisi, cerrahi olarak düzeltilmesi güç, nispeten yaygın bir anomalidir. Erken dönemde işitme du¬rumunu ve atrezi derecesini değerlendirmek gerekir. Geçen 8 yılda 13 konjenital kulak atrezili vakaya transmastoid yakla¬şımla 13 ameliyat yapıldı. Hastalar 6 ay ile 8 yıl arasında takip edildi. 3 vaka dışındaki 10 vaka halen takip edilmektedir. Bu 10 vakadan 9'unda değişik derecelerde işitme kazancı oldu. Fasial paralizi ve sensorinöral işitme kaybı gibi bir problem olmadı. En yaygın komplikasyon %30 oranla dış kulak yolu stenozuydu. Mastoid kaviteyi örtmek içinfull-thickness cilt grefti yerine split-thickness greftin kullanılması stenoz insidansım azaltmaktadır. Bu makale atrezi cerahisinin tarihi gelişimini gözden geçirmekte ve ameliyat öncesi değerlendirmeler ile postoperatif' komplikasyonları tartışmaktadır.
Abstract
Congenital aural is a relatively common anomaly that is difficult to surgically correct. It requires an early as-sesment of hearing status and an early evaluation of the degree of atresia. During the past 8 years, 13 patients with congeni¬tal aural atresia underwent 13 operations by transmastoid approach. Follow-up ranged from 6 months to 8 years. Ten cases have been followed except for three. Nine often patients showed some improvement in hearing. There were no instances of facial nerve inwury or sensorineural hearing loss. The most ferquent complication was stenosis of the external auditory canal, with an incidence of 30%. The use of split-thickness skin graft to cover the mastoid cavity has decreased the incidence of stenosis. This paper will review the historical development of atresia surgery, and discuss the preoperative evaluations and the postoperative complications.
