Özet

Günümüzde, tümörlerin klinik davranışları üzerindeki etkileri olduğu ileri sürülen genetik ve immünolojik genetik markerler larinks kanserli hastaların evrelelendirilmesi veya tedavisinde göz önüne alınmamaktadır. Biz, immünohistokimyasal olarak CD34 ve p53 larinks kanserli 41 hastada çalıştık ve bunların lenf nodu metastazı, tümör rekürrensi, histopatolojik diferansiasyon derecesi ve lenfoplazmositer inflamasyon ile ilişkilerini araştırdık. P53 hastaların %22.2'sinde pozitifken, CD34 %25'9'unda pozitifti. Hastaların 7.4'ünde hem CD34, hemde p53 pozitifti. Lenf nodu metastazı olan 10 hastanın hiçbirinde p53 ve/veya CD34 pozitif değildi. Bu hastaların 8'inde (%80), tümör çevresinde kuvvetli lenfoplazmositer inflamasyon bulunduğunu gözledik. Bu sonuçlara göre, eğer primer tümör CD34 ve p53 negatifse ve tümör çevresinde zayıf lenfoplazmositer reaksiyon varsa, lenfatik metastaz, riskinin az olabileceğini öne sürebiliriz. P53'ün orta derecede veya kötü diferansiye tümörlerde daha yüksek oranda pozitif olduğunu tespit ettik. Iki hasta dışında, CD34 ve/veya p53 pozitif olan bütün hastalarda (%90.2) tümöral inflamasyonunun zayıf derecede pozitif olduğunu tespit ettik. (p<0.01). Rekürrensi olan sadece 2 hastamız (%3.7) vardı ve bunların ikisi de p53 ve CD34 negatifti. Nüks eden hasta sayısı az olduğu için istatistiki test uygulanamadı.

Abstract

Currently, the genetic and immunologic markers, which are suggested to have an influence on tumoural behavior are not taken into consideration in staging and treatment of laryngeal carcinoma. We studied expression of CD34 and p53 immonushistochemically, and investigated their relation to nodal metastasis, tumoural recurrence, histopatological grading, and lymphoplasmocytic infiltration of the tumour on 54 patients with laryngeal carcinoma. P53 was positive in 22.2 % of the patients while CD34 was positive in 25.9 % .In 7.4 % they were co-expressed. None of the 10 patients with nodal metastasis were positive for either p53 or CD34. We noted strong lymphoplasmocytic inflammation surrounding the tumour in 8 of these 10 patients (80%). We may suggest that if the primary tumour is p53 and CD34 negative, and there is a weak inflammatory reaction surrounding the tumour, lymphatic metastasis may be unlikely. It was noted that thep53 was positive in a higher proportion of the patients with modaretely or poorly differentiated tumours. In all but two CD34 and/or p53 positive patients (90.9%), inflamation was veakly positive (p<0.01). We had only 2 patients who recurred in the neck (3.7%). Both of these patients had p53 positive and CD34 negative tumours. Since the number of patients who recurred was small, stastical analysis could not be performed.
