Özet

Esas olarak bir çocukluk çağı hastalığı olmakla birlikte erişkinlerde de görülebilen sekretuar otitis medianın (SOM) gelişiminde rol oynayan nedenler tartışmalıdır. SOM'lı nazal ve nazofarengeal patolojiler araştırılmıştır. Gereç ve Yöntem: Mart 1997 - Ekim 1998 tarihleri arasında polikliniğimizde ilk kez SOM tanısı olan 42 erişkin hasta çalışmaya alındı. Hastalar 17-84 yaşları arasındaydı (ortalama yaş 50 16.4). erkeklerin kadınlara göre oranları 28/14 idi. Rutin KBB muayene sonrası yapılan endoskopik incelemede sağ ve sol toplam 84 nazal kavite nazofarenks değerlendirildi. Bulgular: 22 nazal kavite (%26.1) ve 30 nazofarenks (%35.7) muayenesi normaldi. Nazal kavitenin 32'de pürülan akıntı (38.0), 20' de septum deviasyonu (%23.8), 12'de orta konka hipertrofisi (%14.2), 9 'unda alt konka hipertrofisi (%10.7), 6'sında seröz burun akıntısı (%7.1), 2'sinde antrokoanol polip (%2.3) saptandı.Östaki tüpü ağızlarının 26'sında pürülan akıntı (%30.9), 18'inde tıkanıklık (%21.4), 10'unda ödem (%11.9), 4'ünde atrofi (%4.7) gözlendi.Kırk iki hasanın 18'inde sinüzit (%42.8), 2'sinde nazofarenks karsinomu (%4.7), 2'sinde antrokoanol polip (%4.7), 1'inde radyoterapi (% 2.3), 1'inde adenoid doku hipertrofisi (%2.3), 1 'inde kafa travması (%2.3) olası SOM nedenleri olarak değerlendirilirken, 17(%40.4) hastada belirgin bir neden saptanmadı. Sonuç: Bulgularımız daha önce vurgulana SOM ve sinüzit arasındaki ilişkiyi doğrular niteliktedir. Nazal endoskopik sinüzit tanısında oynayacağı anahtar rolün yanı sıra nazofarenks karsinomu gibi patolojilerin erken tanısında olanak vermesi nedeniyle SOM'lı hastaların araştırmasında önemli bir yer tutmalıdır.

Abstract

Introduction: Although secretory otitis media (SOM) is primarily a childhood problem, it can also be found in adults and the factors involved in development of this disease is controversial. Among the risk factors of SOM are nasal and nasopharyngeal pathologies. In this prospective study, the nasal and nasopharyngeal findings in adult patients with SOM were investigated. Materials and Methods: Between March 1997 - October 1998, 42 adult patients diagnosed as SOM first time in our outpatient unit were included in the study. Patients were between 17-84 years of age (mean age 50 16, 4) and male/female ratio was 28/ 14. After routine ENT examination, totally 84 nasal cavities and nasopharynx were assessed with endoscopic examination. Findings: The examinations of 22 nasal cavities (26.1 %) and 30 nasopharynx (35.7 %) were normal. Thirty-to nasal cavities (38.0%) showed purulend discharge, 20 (23.8 %) septal deviation, 12 (14.2%) hypertrophy of middle conchae, 9 (10.7 %) hypertrophy of inferior conchae, 6 (7.1%) serous nasal discharge and 2 (2.3 %) antrochoanal polyp. At the orifice of Eustachian tubes there were purulent discharge in 8 sides (30.9 %), obstructions in 18 sides (21.6 %), oedema in 10 sides (11.9%) and atrophy in 4 sides (4.7%).The factors possibly causing SOM were appreciated as sinusitis in 18 (42.8) of 42 patients, nasopharyngeal carcinoma in 2(2.3%) and antrochonal polyp in 2(2.3%), whereas no detectable causes found in 17 patients (40.4%), History of radiotherapy, adenoid vegetation and history of head trauma were present in the rest 3 patients. Results: Our findings are comparable with the ones pointing out the relationship between SOM and sinusitis. Because of the fact that nasal endoscopy plays a key role in the diagnosis of sinusitis in addition to its ability of detecting nasopharyngeal carcinoma earlier, it should have an important place in investigation of patients with SOM
