Nazal polipozis tedavisinde oral streoid kullanımı

Özet
Nazal polipozisin tedavisinde bir görüş birliği yoktur. Bu çalışmada 70 nazal polipozisli hasta iki gruba ayrılarak, 35 hastaya 50 mg/gün den başlayıp 13 gün içerisinde azaltılarak kesilen oral prednizolon verildi; 35 hastaya da cerrahi tedavi uygulandı. Her iki gruptaki hastalarin tedavi öncesi ve sonrası burun tıkanıklığı, başağrısı postnasal akıntı ve koku kaybi şikayetleri sorgulanarak, derecelendirildi. Hastaların tedavi öncesi ve sonrası paranazal sinds tomografileri (CT) çektirilerek, evrelendi. Steroid grubunda tedavi öncesi ve sonrasr CT evrelerindeki düzelme istatistiksel olarak anlamlıydi. Her iki tedavi türünün de semptomlar üzerinde etkili olduğu, başağrısı ve postnasal akıntı açısindan steroid tedavisi ve cerrahi tedavinin birbirlerine karşı üstünlüklerinin olmadığı görüldü. Burun tıkanıklığını kontrol etmede cerrahi tedavinin, koku kaybını azaltmada ise oral steroid tedavisinin daha başarılı olduğu ortaya çıktı.
Abstract
There is no consensus on the treatment of nasal polyps. In this study, 70 patients with nasal polyps were divided into two groups. Thirty-five patients ni the "steroid group" received prednisolone 50 mg/day p.o. that was tapered off with 5 mg daily. Thirty-five patients in the "Surgery group" had surgery for their nasal polyps. Patients in both groups were questioned for nasal blockage, headache, postnasal drip and loss of smell before and after treatment, and their symptoms were scored. Computed tomographic scans (CT) of the paranasal sinuses were obtained and staged before and after the treatment. The recovery of the polyps on the CT was sta-tistically significant in the group. Both steroid therapy and surgery significantly reduced all symptoms they were equally effective on controlling headache and postnasal drip. Surgery was more effective on controlling nasal blockage whereas oral steroid treatment was more efeective on controlling nasal blockage whereas oral steroid treatment was more efeective on the treatment of the loss of smell.
