Özet
Akut Baş ve boyun defektlerinin onarımındaki'güçlük, yöresel özellikleri nedeni ile plastik cerrahi için oldukça önemli konu olmayı, mikrocerrahi tekniklerinin gelişmişliğine rağmen halen sürdürmektedir. Geniş rotasyon arkı ile pekto¬ralis major kas-deri flebi, gerek tek gerekse çift deri adası taşınmak suretiyle yörenin tam ya da yarı kalınlık defektlerinin kapatılmasında plastik cerrahın öncelikli seçeneklerinden biridir. GATA Plastik ve Rekonstrüktif Cerrahi Anabilim Dalı'nda son dört yılda dokuz tümör, beş ateşli silah yaralanması ve bir boyun önünde elektrik yanığına bağlı açık yarası olan toplam onbeş olguda pektoralis majör kas-deri flebi ile onarım uygu¬lanmış, bu olgulardan üçünde osteomüskülokütan, diğerlerinde müskülokütan flep kullanılmıştır. Altı olguda tam kat defekt nedeni ile çift deri adalı flep kullanılırken dokuz olgunun def ekti tek deri adası ile kapatılmıştır. Yalnız bir olguda total deri adası kaybı oluşmuş, bir olgudaki parsiyel yüzey el kayıp ise spontan iyileşmiştir. Beşinci kostanın birlikte taşındığı tüm ol¬gularda erken ya da geç dönemlerde kemiğe ilişkin çeşitli komplikasyonlar görülmüştür.
Abstract
AcuteDifflculty of the reconstruction of head and neck deformities does still keep its importance for the plastic surgery in spite of the advances in microsurgical techniqes. Both single and double skin paddled pectoralis major flaps with their wide arc of rotation have a priority to be selected by plastic surgeon to close the full or partial thickness defects of head and neck region. During last four years, in our department fifteen pectoralis major flaps have been used to reconstruct the defects in nine pati¬ents with malignant tumors, in five with firearm injuries and in one with open waunddue to electrical injury. All flaps were myocutaneous and three of them have included costal bone. Flaps in six patients have been used with double skin island to cover the full thickness defects, the other defects have been closed with single skin paddled pectoralis major myocutaneous flaps. In one patient single skin island has been totally lost, in another patient superficial skin necrosis has occured on the distal portion of the flap, but it has spontaneously recovered. During postoperative period, early or late, some bone related compli¬cations have been seen in all osteomyocutaneous flaps.
