Özet
Paranazal sinüs infeksiyonlarının çoğunlukla ostiomeatal kompleksteki stenozlar sonucu oluştuğu, maksiller ve frontal sinüzitlerin tamamen bu bölgedeki stenozlara bağlı olarak sekonder olduğu tespit edilmiştir. Dar olan bu bölgedeki inflamasyon ve anatomik varyasyonların yarattığı stenoz, mukosilier aktiviteyi dolayısıyla sinüslerin drenaj ve ventilasyonunu bozarak kronik inflamatuar paranazal sinüs hastalıkları için uygun zeminin oluşmasına neden olmaktadır. Ostiomeatal kompleksteki anatomik varyasyonların, bölgedeki stenozların en önemli nedeni olduğu bilinmektedir. Bu makalede, endoskopik sinüs cerrahisi uyguladığımız 415 hastada ve asemptomatik 60 kişilik kontrol grubunda anatomik varyasyonların görülme oranları karşılaştırmalı olarak değerlendirilmekte, anatomik varyasyonların paranazal sinüs infeksiyonlarındaki rolü tartışılmaktadır
Abstract
It has been proven that most of the paranasal sinus infections result from the stenoses of the ostiomeatal complex, and maxillary and frontal sinusitis are completely secondary to these stenoses at the ostiomeatal complex. Stenoses are brought about by inflammation and anatomic variatinos is this already narrow region, which hamper the mucociliary activity and consequently the drainage and ventilation of the sinuses. Hence, the suitable milieu for chronic inflammatory paranasal sinus disease is created. Anatomic variations at the ostiomeatal complex are known to be the most important cause of stenoses in this region. In this article, the incidences of anatomic variations are comparatively determined in both the 415 patients on whom we performed endoscopic sinus surgery and the control group of 60 persons who did not have any symptoms pertaining to sinusitis. The role of anatomic variations in paranasal sinus infections is discussed.
