Özet

Amaç: bu çalışma çocukluk çağında en sık işitme kaybı nedenleri arasında bulunan effüzyonlu otitis mediada da görülen enflamasyonunun üst solunum yolu epitelinde yaygın olduğu düşünülerek, bu vakaların burun mukozalarındaki enflamatuvar değişikliklerin ultrastrüktürel düzeyde gösterilmesi için planlandı. Gereç ve Yöntem: Bu çalışmada 15 effüzyonlu otitis medialı ve 10 normal çocuğun orta konkalarından alınan biyopsiler elektron mikroskop ile incelendi. Bulgular: Hasta grubunda, kontrol grubundaki çocukların bulguları ile karşılaştırıldığında, yüzey epitelinde ileri derecede dejenerasyon, silyalı ve mikrovilluslu hücre sayısında azalma, hücreler arası mesafede genişleme ve silyalı hücrelerin sekretuvar fonksiyon kazandıkları saptandı.Sonuç: effüzyonlu otitis medialı hastaların nazal kavitelerinde demonstre edilen enflamatuvar değişiklikler, enflamasyonun orta kulağa kısıtlı kalmadığını, burun mukozasında da yaygın olduğunu ortaya koymaktadır.

Abstract

Aims: Otitis media with effusion is a frequent cause of hearing loss in the pediatric population and it effects the entire upper respiratory tract. In this study we showed inflammatory changes of the nasal mucosa by electron microscope in children with otitis media with effusions. Materials and methods : Biopsy specimens of the nasal mucosa from the middle turbinates of 15 children with otitis media with effusion were examined by electron microscopy and the results were compared with those from 10 healthy children. Findings: Compared to the healthy subjects electron microscopic examinations of biopsy specimens showed degeneration of epitelium, reduction in the number of ciliated cells, widening of intercellular space and beginning of secretary function in the ciliated cells. Results: Inflammatory changes of the nasal cavity in patients with effusions showed that these changes are not only limited to nasal cavity in patients with effusions showed that these changes are not only limited to the middle ear but also spreads to the entire upper respiratory tract.
