Özet

Amaç: Nazal septum deviasyonu (NSD) ve internal nazal valv kollapsında (INVK) akustik rinometri(AR), rinomanometri (RMN) ve “Nose Obstruction Symptom Evaluation (NOSE)” skalasınınpreoperatif ve postoperatif tanı yöntemi olarak kullanılıp kullanılamayacağını göstermektir.Gereç ve Yöntemler: Bu çalışma 31 NSD, 36 INVK’li hasta ile prospektif olarak gerçekleştirildi.Ameliyat kararı verilen toplam 67 hastaya preoperatif AR ve RMN yapıldı. Aynı zamanda, “BurunTıkanıklığı Semptom Değerlendirilmesi [Nose Obstruction Symptom Evaluation (NOSE)]” Skalasınıdoldurmaları sağlandı. NSD tanısı alan 31 hastaya septoplasti, NSD-INVK tanısı alan 36 hastaya iseseptoplasti ve bilateral nazal valf cerrahisi uygulandı. Bulgular: Ameliyat sonrası her 2 grupta NOSEskalasında istatistiksel olarak anlamlı bir düşüş görüldü. AR ölçümlerine göre minimal kesit alanıve nazal hacimde artış kaydedildi. RMN ölçümleri, nazal rezistansta azalma ve nazal hava akımındaartış gösterdi (p=0,00). Ayrıca, hem ameliyat öncesi hem de ameliyat sonrası NOSE skalası, AR veRMN değerleri arasında korelasyon vardı. Sonuç: AR ve RMN, hem NSD ve INVK’ye bağlıobstrüksiyonu hem de postoperatif başarıyı değerlendirmede objektif, NOSE skalası ise subjektiftanı yöntemi olarak kullanılabilir.

Abstract

Objective: This study was performed to determine whether acoustic rhinometry (AR), rhinomanometry (RMN) and Nose Obstruction Symptom Evaluation (NOSE) scale can be used as preoperative and postoperative diagnostic methods in NSD and INVK. Material and Methods: This study was performed prospectively with 31 patients with nasal septum deviation (NSD) and 36 patients with internal nasal valve collapse (INVK). Preoperative AR and RMN were performed in 67 patients. Patients were asked to complete the NOSE scale. 31 patients with NSD underwent septoplasty, 36 patients with NSD-INVK underwent septoplasty and bilateral nasal valve surgery. Results: There was a statistically significant decrease in NOSE scale in both groups postoperatively. Increased minimal cross-sectional area and nasal volume were recorded according to AR measurements. RMN measurements showed a decrease in nasal resistance and an increase in nasal airflow (p=0.00). In addition, there was a correlation between both preoperative and postoperative NOSE scale, AR and RMN values. Conclusion: AR and RMN can be used as an objective diagnostic method to evaluate both NSD and INVK related obstruction and postoperative success, and NOSE scale can be used as a subjective diagnostic method.
