Özet

Amaç: Bu çalışmada temel sağlık hizmetleri yürütmekte olan hekimlerin akut bakteriyel tonsillit (ABT) ve viral üst solunum yoluenfeksiyonu (ÜSYE) değerlendirme ölçütlerine günlük uygulamalarında ne oranda önemli olduğu belirlenmesi amaçlandı.Gereçler ve Yöntem: Bu çalışma yüz yüze anket yöntemi ile Nisan 2010-Mayıs 2010 tarihleri arasında yapıldı. Önceden belirlenen ABTpatogenez, tanı ve tedavi ölçütleri ile ilgili soruları 1-5 arasında derecelendirilerek yanıtlamaları istendi. Ayrıca Hekimlerin demografik velisans eğitim bilgileri, günlük gördükleri hasta sayısı, ABT viral ÜSYE tanısı konan hasta sayısı ve aile hekimi olup olmadıkları sorgulandı.Bulgular: Çalışma sırasında il merkezinde 21 Aile Sağlık merkezi veya Toplum Sağlık Merkezinde 91 Hekim ziyareti gerçekleştirildi.Anketi yanıtlamayı 66 hekim kabul etti. Anketi yanıtlayanların cinsiyet dağılımı % 62,1 erkek, % 37,9 kadın, yaş ortalaması 45,97±5,1 yılolarak saptandı. Hekimlik süresi 21,26±5 yıl, % 52,2 aile hekimi ve % 47,8 pratisyen olduğu saptandı. Tanı koymada öykü (% 69,7) vekültür (%86,4) en çok önem veirlen tanı ölçütü olduğu ifade edildi. Antibiyotiklerden, Amox-clu (%87,9), peroral penislin (%69,7) veparenteral pensilin (%42,4) oranında tercih edildiği saptandı.Sonuçlar: Ziyaret edilen hekimlerin % 72,5' i anketi yanıtlamayı kabul etti. Yapılan anket sonucunda Temel sağlık hizmetlerininyürütülmesinde görev alan Pratisyen ve Aile Hekimlerinin Akut bakteriyel tonsillit ve viral ÜSYE değerlendirme ölçütlerinin çoğunluklagüncel rehberlerle uyumlu olduğu saptandı. Ancak Centor Skorlaması tanı ölçütlerinin tam olarak benimsenmediği hatta çok az oranda tamolarak benimsendiği saptandı. Mezuniyet sonrası eğitim kapsamında akut tonsillit ve viral ÜSYE tanı ve tedavi eğitimlerinin yer almasınınyararlı olacağı düşünüldü.

Abstract

Objective: The aim of this study was to determine the criteria for acute bacterial tonsillitis (ABT) and viral upper respiratory tract infection (URI) among family medicine practioners in their daily practice. Materials and Methods: This study was conducted between April 2010 and May 2010 by face to face questionnaire. The doctors were asked to answer questions about predetermined ABT pathogenesis, diagnosis and treatment criteria by grading 1-5. Demographic and education information, daily number of patients, number of patients diagnosed as ABT viral URI and whether they were family physicians were also questioned. Results: During the study, 91 physician visits were made in 21 Family Health Centers or Community Health Centers in the city center. Sixtysix physicians accepted to answer the questionnaire. The gender distribution of the respondents was 62.1% male, 37.9% female, and the mean age was 45.97 ± 5.1 years. Duration of the study was 21.26 ± 5 years, 52.2% were family physicians and 47.8% were general practitioners. History (69.7%) and culture (86.4%) were the most important diagnostic criteria for diagnosis. Among the antibiotics, Amoxclu (87.9%), per oral penicillin (69.7%) and parenteral pencil (42.4%) were preferred. Results: 72.5% of the physicians visited agreed to answer the questionnaire. As a result of the questionnaire, it was determined that the criteria of Acute bacterial tonsillitis and viral URIs of General Practitioners and Family Physicians involved in the execution of primary health care services were mostly in line with current guidelines. However, it was found that the diagnostic criteria of the Centor Scoring were not fully adopted or even fully adopted. Acute tonsillitis and viral URI diagnosis and treatment trainings were considered to be beneficial in the postgraduate education
