Özet

Amaç: Bu çalışmada üçüncü basamak bir üniversite hastanesinin kulak burun boğaz (KBB) anabilim dalına ait klinik ve poliklinik maliyetleri hesaplanarak hastane yöneticilerine rehberlik sağlanması amaçlandı. Gereç ve Yöntemler: 01 Ocak 2013 - 31 Aralık 2013 tarihleri arasında KBB anabilim dalına ait tıbbi, idari ve mali kayıtlar gerekli izinler alınarak incelendi ve değerlendirildi. Çalışmanın evreni belirtilen bir yıl içinde KBB anabilim dalı kliniğinde yatarak ve ayaktan hizmet alan tüm hastaları içerdi. Bulgular: 2013 yılında KBB anabilim dalı polikliniğine toplam 35.195 hasta başvurdu. Bu hastalardan 2.243ü yatarak tedavi edildi ve toplam 3.104 ameliyat gerçekleştirildi. Bu süre içerisinde KBB anabilim dalı için toplam 4.997.673 ¨ harcama yapıldı. Toplam maliyetin %44.83ü personel giderlerini, %35.86sı ilaç ve tıbbi malzeme giderlerini, geri kalan %19.31i ise yemek, temizlik, elektrik su doğalgaz ve güvenlik hizmet alımı gibi diğer giderleri içerdi. Sonuç: 2013 yılında yapılan toplam harcamaların büyük çoğunluğu klinikte gerçekleşmiştir. Sağlık yöneticileri hastane maliyetlerini düşürmek için maliyet analizi çalışmalarına önem vererek klinik ve poliklinik maliyetlerini belirlemeli ve giderleri arasındaki ilişkileri tespit etmelidir.

Abstract

Objectives: This study aims to provide guidance to hospital administrators by calculating clinic and polyclinic costs of ear, nose and throat (ENT) department in a tertiary university hospital. Materials and Methods: Medical, administrative and financial records of ENT department between January 01st 2013 and December 31st 2013 were examined and evaluated upon receiving the required permissions. The universe of the study consisted of all inpatients and outpatients who took service at ENT department during the indicated year. Results: A total of 35,195 patients admitted to the polyclinic of ENT department in 2013. Of these patients, 2,243 received inpatient treatment and a total of 3,104 surgeries were performed. During this period, a total of 4,997,673 ¨ expenditure was made for ENT Department. Of the total cost, 44.83% included personnel expenses, 35.86% medications and medical supplies expenses, and the remaining 19.31% other expenses including food, cleaning, electricity water gas, and security services procurement. Conclusion: The majority of the total spending made in 2013 has been in the clinic. To reduce hospital costs, health administrators must determine clinic and polyclinic costs by prioritizing cost analysis studies and identify the relationships between expenses.
