Van Gölü suyunun normal orta kulak mukozasına etkisi: Tavşanda deneysel çalışma

Özet
Amaç: Kulak zarı bütünlüğünün bozulduğu ancak orta kulak mukozasının sağlam olduğu olgularda Van Gölü suyunun etkileri tavşan modeli üzerinde araştırıldı. Çalışma Planı: Herhangi bir kulak hastalığı belirlenemeyen New Zealand türü 16 tavşandan dört grup oluşturuldu. Sıvı uygulaması için ayrılan kobayların kulak zarları anestezi altında perfore edildi ve gruplara göre bir hafta süreyle günde iki kez normal göl suyu, steril edilmiş göl suyu ve steril serum fizyolojik uygulandı. Bir grupta hiçbir işlem yapılmadı. Tüm kobayların yaşamları sekizinci günde aşırı anestezi verilerek sonlandırıldı. Orta kulak mukozaları patolojik incelemeye alındı. Mukoza epitelinde sıralanma artışı, mukozada meydana gelen kalınlık artışı, damar dışına çıkan eritrosit ve nötrofil lökosit varlığı değerlendirildi. Bulgular: Normal ve steril edilmiş göl suyu uygulanan gruplardaki histopatolojik değişiklikler kontrol gruplarına göre anlamlı farklılık gösterdi. Normal göl suyu verilen grupta incelenen parametreler steril göl suyu uygulanan gruba göre çok belirgin olmasına rağmen, aralarında anlamlı farklılık bulunmadı (p>0.05). Serum fizyolojik grubunda ise mukozal kalınlıkta hafif artış ve çok hafif miktarda nötrofil lökosit infiltrasyonu izlendi. Sonuçlar: Tavşan orta kulak mukozasındaki değişikliklerde, göl suyu bileşiminin bakteriyolojik kirlilikten daha fazla etkili olduğu sonucuna varıldı.
Abstract
Objectives: In a rabbit model, we investigated the effects of Lake Van water under conditions in which the eardrum was perforated leaving the middle ear mucosa intact. Study Design: Sixteen New Zealand rabbits without any ear problems were assigned to four groups, three of which were treated with normal lake water, sterilized lake water, and sterile saline solution, respectively, twice daily for a week after induction of ear drum perforations under local anesthesia. The forth group was left untreated. On the eighth day, all the animals were sacrificed by over-dose anesthesia for histopathologic examination of the middle ear mucosa. Mucosal changes were rated including thickness, extravasation of erythrocytes, infiltration of neutrophil leukocytes, and increase in mucosal lining. Results: The histopathologic mucosal changes in groups receiving sterile and non-sterile lake water differed significantly from those of the control groups. Although all the parameters examined were more evident in the non-sterile lake water group, the differences from the sterile lake water group did not reach signifi-cance (p>0.05). In the sterile saline group, a mild increase in mucosal thickness and minimal infiltration of neutrophil leukocytes were observed. Conclusion: The effects observed on rabbit middle ear mucosa seem to arise from the composition, rather than the bacteriological pollution of the lake water.
