Özet

Amaç: Semer burun (saddle nose) deformitesi nedeniyle seçilmiş hastalarda uyguladığımız flying wings tekniği ile onarımın sonuçları değerlendirildi. Hastalar ve Yöntemler: Supratip semer burun deformitesi belirlenen beş hastada (4 kadın, 1 erkek; ort. yaş 28; dağılım 22-34) flying wings tekniği ile onarım yapıldı. Deformitelerin nedeni iki hastada burun travması, üç hastada geçirilmiş septoplasti ameliyatı idi. Hastalarda septal desteğin tama yakınının kaybolmuş olduğu görüldü. Hastalara flying wings prosedürü ile birlikte kostal kartilaj greft, septoplasti, kolumella strut grefti uygulandı. Cerrahi tekniğin sonuçları hastaların memnuniyeti, ameliyat sonrası çekilen fotoğraflar ve nazal dorsum yapısının incelenmesi ile değerlendirildi. Hastalar ameliyattan sonra en az altı ay (dağılım 6 ay-1 yıl) süreyle izlendi. Bulgular: Tüm hastalar cerrahi sonuçlarından memnuniyet duyduklarını belirtti. Bir hastada ameliyattan beş ay sonra rhinion bölgesinde oluşan düşüklük ikinci bir ameliyatla düzeltildi. Sonuç: Flying wings prosedürü semer burun deformitelerinin tamirinde, özellikle septal kıkırdak desteklerin tümüne yakınını yitirmiş hastalarda kullanılabilecek uygun bir yöntemdir.

Abstract

Objectives: We evaluated the results of reconstruction of saddle nose with the use of the flying wings technique in selected patients. Patients and Methods: Five patients (4 women, 1 man; mean age 28 years; range 22 to 34 years) with supratip saddle nose deformity underwent reconstruction with the use of the flying wings tech-nique. The deformity resulted from traumatic events in two patients and from previous septoplasty operations in three patients. All the patients had an almost total loss of septal support. Additional procedures employed were costal cartilage graft, septoplasty, ana columellar strut graft. The results of surgery were assessed by the degree of patient satisfaction, postoperative photographic analysis, and evaluation of the nasal dorsum. The mean follow-up period was at least six months (range 6 months to one year). Results: All the patients were satisfied with post-operative results. One patient required a subsequent operation five months after surgery because of collapse at the rhinion. Conclusion: The flying wings procedure is a con-venient method in the reconstruction of saddle nose deformity especially in patients who exhibit a nearly total lack of septal cartilage support.
