Özet
Aktinomikozis, Actinomyces israelii' nin neden olduğu, birçok dokuyu tutan, seyrek görülen kronik, granülomatöz bir enfeksiyondur. Sıklıkla servikofasyal bölgeyi tutar. Bu yazıda, 47 ve 20 yaşlarında ve submandibuler ve tonsil aktinomikozu görülen iki erkek hasta sunuldu. İlk hasta, boynun sağ tarafında üç aydır var olan şişlik ile başvurdu. Diğer hasta ise üç yıldır yüksek ateşle seyreden tekrarlayan tonsillit (5-6 kez/yıl) atakları geçirmekteydi. İlk olguda submandibular gland ve çevre dokularda yapışıklıklara neden olan multipl apse odaklan içeren kitle bütün olarak çıkarıldı. İkinci olguda kronik tonsillit düşünülerek bilateral tonsillektomi yapıldı. Patolojik tanı kronik aktinomikoz tonsilliti olarak kondu. Bu olguda, ameliyat sonrası dokuzuncu ve on yedinci günlerde sağ tonsil üst polünde enflamasyonla birlikte sızıntı tarzında reküren hemoraji gözlendi.
Abstract
Actinomycosis is a rare, chronic, granulomatous infection due to Actinomyces israelii, affecting several tissues. It is most often seen in the cervicofacial region. We encountered submandibular and tonsil actinomycosis in two male patients with ages 47 and 20 years, respectively. The first patient presented with swelling on the right side of his neck while the latter patient had recurrent tonsillitis accompanied by high fever. Surgical resection was performed in the first patient, together with multiple abscesses causing adhesions in the submandibular gland and adjacent tissues. The other patient underwent bilateral tonsillectomy following an initial diagnosis of chronic tonsillitis. Pathologic examination revealed chronic tonsil actinomycosis. The patient exhibited recurrent hemorrhages in the upper pole of the right tonsil after nine and 17 days of operation.
