Özet

Amaç: iletim tipi işitme kaybında kemik yolu ile uygulanan işitsel beyinsapı yanıtları (ABR) odyometrisi önerilen, ancak tartışmalı bir tekniktir. Bu çalışmada, kemik vibratör ve kulaklık ile üretilen filtre klik ve ton burst şeklindeki işitsel uyaranların akustik analiz sonuçları karşılaştırıldı. Çalışma Planı: 500,1000,2000 ve 4000 Hz frekanslarda, her bir frekans için 11/sn ve 31/sn'lik uyaran hızlarında, kulaklık ve kemik vibratörden verilen filtre klik ve ton burst şeklindeki işitsel uyaranlar dinamik mikrofon aracılığı ile bilgisayara aktarıldı ve ses analizleri gerçekleştirildi. Bulgular: Kemik vibratör ile oluşan filtre kliklerin akustik spektrogramlarında 2400 Hz'de, pes frekanslardaki ton burstlerde ise 2200-2300 Hz'de pik oluştu. Kemik vibratör kaynaklı tiz frekanslardaki ton burstlerde, kulaklıktan elde edilen pes frekanslardaki filtre kliklerde ve tiz frekanslardaki ton burstlerde, belli bir frekansta pikin oluştuğu net bir spektrogram elde edilemedi. Kulaklık kaynaklı tiz frekanslardaki filtre klikler ve pes frekanslı ton burstlerde ise birden fazla frekans bölgesinde piklerin oluştuğu akustik spektrogram gözlendi. Sonuç: Kemik yolundan ve, benzer şekilde, hava yolundan uygulanacak işitsel uyaranların akustik spektrumları, varsayılan frekanslarından farklılıklar göstermektedir. Bu sonuç, kemik yolu ile ya da hava yolu ile uygulanan frekansa spesifik ABR odyometrisinin sübjektif niteliğini ortaya koymaktadır.

Abstract

Objectives: Bone-conducted auditory brainstem response (ABR) audiometry is a proposed, but debatable technique in cases with conductive hearing loss. This study was designed to compare the results of the acoustic analyses of acoustic stimuli generated by an earphone and bone vibrator. Study Design: Earphone- and bone vibrator-generated filtered clicks and tone bursts at 500, 1000, 2000 and 4000 Hz, with stimulation rates of 11/sec and 31/sec were recorded and transferred to a computer where the acoustic analysis were performed. Results: In the acoustic spectrograms of bone vibrator-generated filtered clicks, an acoustic peak was formed at 2400 Hz, whereas, for tone bursts in low frequencies, the peak value was obtained at 2200-2300 Hz. Bone vibrator-generated tone bursts in high frequencies, earphone-generated filtered clicks in low frequencies and tone bursts in high frequencies failed to produce a spectrogram with a peak formation at any frequency. Earphone-generated filtered clicks in high frequencies and tone bursts in low frequencies showed acoustic spectrograms of multiple peaks, with peak formation in more than one frequency. Conclusion: Bone vibrator- and earphone-generated auditory stimuli have different acoustic spectra with respect to their assumed frequency values. These results emphasize the subjectivity of the frequency specific bone-conducted or air-conducted ABR audiometry.
