Baş-boyun kanserlerinin rekonstrüktif cerrahisinde pektoralis majör myokütan flep uygulamalarımız

Özet
Amaç: Baş ve boyunda oluşan defektlerin rekonstrüksiyonunda kullandığımız pektoralis majör myokütan flebinin etkinliğini ve güvenilirliğini değerlendirmek. Hastalar ve Yöntemler: Kliniğimizde baş-boyun kanseri nedeniyle pektoralis majör myokütan flep ile rekonstrüksiyonu yapılan 22 hasta (19 er-kek, 3 kadın; ort. yaş 50; dağılım 20-65) tümör yerleşimi ve histopatolojik tipi, evresi, uygulanan cerrahi tedavi, rekonstrüksiyon şekli ve komplikasyonlar açısından değerlendirildi. Bulgular: On iki olguda (%54.5) komplikasyon görüldü. Komplikasyonlarm çoğu önemli nitelikte değildi ve konservatif yöntemlerle tedavi edilebildi. Total flep nekrozuyla hiç karşılaşılmadı. Bir olguda ağır olmak üzere dört olguda (%18.2) parsiyel nekroz gözlendi. Sonuç: Karşılaşılan komplikasyonların çoğunun konservatif yöntemlerle tedavi edilebilir olması, pektoralis majör myokütan flebinin oldukça güvenilir bir rekonstrüksiyon materyali olduğunu gösterdi.
Abstract
Objectives: We evaluated the effectiveness and reliability of the pectoralis major myocutaneous flap used in the reconstruction of head and neck defects. Patients and Methods: Twenty-two patients (19 men, 3 women; mean age 50 years; range 20 to 65 years) who underwent reconstruction with the pectoralis major myocutaneous flap for head and neck defects were evaluated. Localization of the tumour, histopathologic type, stage, surgical treatment, reconstruction method, and complications were assessed. Results: Complications were encountered in 12 patients (54.5%), the majority of which was of minor type and treatable with conservative methods. Total flap necrosis did not occur. Partial necrosis developed in four patients (18.2%), one of which was severe. Conclusion: The fact that the majority of complications can be treated with conservative methods makes the pectoralis major myocutaneous flap a reliable reconstruction material in head and neck defects.
