Özet

Tüberküloz parotitis nadir karşılaşılan bir klinik durumdur. Enfeksiyöz ajan, parotise duktal, hematojen veya parotis içindeki lenf nodları yoluyla ulaşır. Bu yazıda, son beş yıl içinde parotiste şişlik şikayetiyle başvuran ve tüberküloz parotitis tanısı konan dört olgu (3 kadın, 1 erkek; yaş dağılımı 42-56) sunuldu. Olguların hiçbirinde akciğerde tüberküloz saptanmadı. Üç olguda sınırlı derecede parotis içindeki bir lenf nodunda ve komşu bez parenkiminde granülomatöz enfeksiyon saptanırken, bir olguda difüz bez tutulumu ve iki odaktan cilde fistülizasyon görüldü, iki olguda tanı ince iğne aspirasyonu, birinde yüzeyel parotidektomi, birinde ise fistülize olmuş kitlenin fistül ağzından alınan biyopsi ile kondu. Parotis kitlesinin ayırıcı tanısında tüberküloz parotitis de akılda tutulmalıdır. Tanıda ince iğne aspirasyon sitolojisinin önemli bir yeri vardır. Bu incelemenin tanısal olmaması durumunda, tanı ancak yüzeyel parotidektomi sonrasında konabilmektedir.

Abstract

Tuberculous parotitis is a rare clinical entity. Mycobacterial disease may have access to the parotid gland through the Stenon duct, hematogenously, or by lymphogenous spread. In this report, four patients (3 females, 1 male; age range 42 to 56 years) with tuberculous parotitis are presented, all were diagnosed within the past five years and had complaints of a mass lesion in the parotid gland. None of them had pulmonary tuberculosis. Three patients were found to have granulomatous infection confined to a single lymph node in the parotid gland and to the adjacent gland parenchyma. One patient had diffuse gland involvement with cutaneous fistulization to the skin through two separate foci. Diagnosis was made by fine-needle aspiration biopsy in two patients, histopathologic examination of the biopsy specimen from the fistula rim in one patient, and of the superficial parotidectomy specimen in the other. Tuberculous parotitis,, should be kept in mind in the differential diagnosis of parotid masses. Fine-needle aspiration cytol-ogy is helpful in the diagnosis; however, if it does not provide diagnostic evidence, then superficial parotidectomy is required.
