Servikal özofagus ve hipofarenks malign tümörlerinde gastrik pull-up ile rekonstrüksiyon

Özet
Amaç: Servikal özofagusu çepeçevre tutan hipofarenjeal veya özofajeal malign tümörlü olgularda, total larengofarengo-özofajektomi (LFÖ) sonrası gastrik pull-up (GP) ile yapılan rekonstrüksiyonun sonuçlan değerlendirildi. Hastalar ve Yöntemler: Çalışmada total LFÖ uygulanan ve GP tekniği ile rekonstrükte edilen altı olguda (3 kadın, 3 erkek; ort. yaş 40; dağılım 27-52) elde edilen onkolojik ve fonksiyonel sonuçlar değerlendirildi. Tümör yerleşim yeri beş olguda poskrikoid bölge, bir olguda servikal özofagus olarak belirlendi. Tümörler iki olguda T2, üç olguda T3, bir olguda T4 evresinde idi. Bulgular: iki olguda gelişen fistül yara iyileşmesine bağlı olarak kapandı. Beş olguda ameliyat sonrası 11-21. günler arasında ağızdan beslenmeye başlandı. Bir olgu, mediastinit ve pnömotoraks nedeniyle ameliyat sonrası 11. günde; diğer bir olgu ise lokal rekürens ve uzak metastaz nedeniyle ameliyattan altı ay sonra kaybedildi. Ameliyat sonrası ikinci yılını dolduran bir olguda peristomal nüks nedeniyle radyoterapi sonrasında kemoterapi uygulandı. Yaşamlarını sürdüren olgularda ameliyat sonrası geçen süre bir olguda altı yıl, iki olguda iki yıl, bir olguda da iki ay idi. Sonuç: Servikal özofagusu çepeçevre tutan hipofarenjeal veya servikal özofajeal tümörlü olgularda total LFÖ sonrası GP ile rekonstrüksiyon tümörün torakal segmente ulaşmadığı olgularda ilk seçenek olabilir.
Abstract
Objectives: We evaluated the results of reconstruction with gastric pull-up (GP) after total laryngopharyngoesophagectomy (LPE) in patients having hypopharyngeal or cervical esophageal malignant tumors with circumferential esophageal infiltration. Patients and Methods: Functional and oncologic results were assessed in six patients (3 men, 3 women; mean age 40 years; range 27 to 52 years) undergoing total LPE followed by reconstruction with the GP technique. The tumors were locat-ed in the postcricoid region in five patients and in the cervical esophagus in one patient. Tumor stages were T2 in two patients, T3 in three patients, and T4 in one patient. Results: Cutaneous fistulae that occurred in two patients were closed by secondary wound healing. Initiation of post-operative oral feeding ranged from 11 to 21 days in five patients. Mortality occurred in two patients; one patient died on the 11th postoperative day due to mediastinitis and pneumothorax, and the other six months after surgery due to local recurrence and distant metastasis. One patient who was alive after two years received chemotherapy after radiation therapy because of peristomal recurrence. Four patients were alive six years (n=l ), two years (n=2), and two months (n=1) after surgery, Conclusion: Total LPE followed by reconstruction with the GP technique may be the first choice of treatment in hypopharyngeal or cervical esophageal tumors with circumferential esophageal infiltration without involvement of the thoracic esophagus.
