Özet
Amaç: Dudak karsinomlarının ortaya çıkışını, semptomlarını, yayılımını ve boyun metastazlarını değerlendirmek. Hastalar ve Yöntemler: Kliniğimizde tedavi edilen dudak karsinomlu 44 hasta (8 kadın, 36 erkek; ort.yaş 60.3; dağılım 30-77) retrospektif olarak değerlendirildi. Bulgular: Ameliyat öncesi histopatolojik tetkikte 42 olguda skuamöz hücreli karsinom, bir olguda bazal hücreli karsinom ve bir olguda da indiferansiye malign tümör saptandı. Skuamöz hücreli karsinomların 24'ü iyi (%57.1), 14'ü orta (%33.3) ve dördü kötü (%9.6) diferansiyasyon göstermekteydi. Yirmi iki olguda (%50) T 1 15'inde T 2 (%34.1), beşinde T 3 (%11.4), ikisinde T 4 (%4.5) tümör bulundu. Klinik boyun muayenesinde 11 hastada palpabl lenfadenopati saptandı (7 hastada N 1 , 2 hastada N 2c , 2 hastada N 3 ). Boyun bilgisayarlı tomografisinde bir hastada mandibulada, kemik sintigrafisinde bir hastada maksillada kemik invazyonu saptandı. Alt dudak kanserlerinde lenf nodu metastaz oranı %19.3 bulundu; üst dudak kanseri olan iki olgunun birinde lenf nodu metastazı görüldü. Sonuç: Dudak kanserlerinde tümör boyutu arttıkça ve diferansiyasyon kötüleştikçe lenf nodu metastazının görülme oranı yükselmektedir.
Abstract
Objectives: We evaluated carcinoma of the lip with regard to presentation, symptoms, extension, and neck metastasis. Patients and Methods: Forty-four patients (8 women, 36 men; mean age 60,3 years; range 30-77 years) with carcinoma of the lip were retrospectively evaluated. Results: Preoperative histopathologic diagnoses were squamous cell carcinoma in 42 patients, basal cell carcinoma in one patient and undifferentiated malignant tumor in one patient. Squamous cell carcinoma was well-, moderately, and poorly differentiated in 24, 14, and 4 patients, respectively. T-staging of the tumors was T<sub>1</sub> in 50% (n=22), T<sub>2</sub> in 34,1% (n=15), T<sub>3</sub> in 11.4% (n=5), and T<sub>4</sub> in 4.5% (n=2). Palpable lymph nodes were detected in 11 patients (N<sub>1</sub> in 7, N<sub>2c</sub> in 2, and N<sub>3</sub> in 2 patients). Computed tormography showed mandibular invasion in one patient and bone scintigraphy detected maxillary involvement in another. Lymph node metastasis was detected in 19.3% of patients with cancer of the lower lip; of two patients with upper lip cancer, one patient developed lymph node metastasis. Conclusion: A high incidence of lymph node metastasis is seen in lip cancers as the tumor size increases and the degree of differentiation becomes worse.
