Özet
Elli altı yaşında ve 18 yıldır diyabetli bir kadın hastaya malign otitis eksterna tanısı kondu. Sağ kulak ağrısı, işitme azlığı ve baş ağrısı şikayetleriyle başvuran hastada oral antibiyotik tedavisi ile yanıt alınamadı. Dış kulak yolu tabanında granülasyon dokusu saptandı. Bilgisayarlı tomografide, sağ mastoid kavitede enfeksiyon ve timpanik kemikte osteit bulguları görüldü. Dış kulak yolundan alınan kültürde Pseudomonas aeruginosa üredi. Seftazidim ile yedi günlük tedaviye yanıt alınamaması üzerine, geniş mastoidektomi yapıldı. Antibiyotik tedavisi dört hafta parenteral sürdürüldü. Altı aylık takibinde hastada herhangi bir komplikasyon gözlenmedi.
Abstract
A fifty-six-year-old female patient was diagnosed as having malignant otitis externa. She presented with pain in the right ear, hearing loss, and headache. She did not respond to oral antibiotic therapy. Examination revealed granulation tissue in the base of external ear. Computed tomography showed infection in the right mastoid cavity and osteitis in the tympanic bone. Cultures obtained from the external ear yielded Pseudomonas aeruginosa. A seven-day treatment with ceftazidime was unsuccessful, so mastoidectomy was performed. Parenteral antibiotic therapy was administered for four weeks. No complications occurred within a follow-up of six months.
