Adenoid doku hacminin östaki tüp fonksiyon testine etkisi

Özet
Amaç: Ostaki tüpünün mekanik ve ekstrensek olarak tıkanıklığına neden olan adenoid kitlesi hacminin Ostaki tüp fonksiyonunu nasıl etkilediği araştırıldı. Çalışma Planı ve Yöntemler: Sadece adenoidektomi veya beraberinde diğer cerrahi yöntemler (tonsillektomi, ventilasyon tüpü yerleştirilmesi) uygulanan 25 çocuğa (yaş ört. 6.7 yıl; yaş dağılımı 2-12 yıl) preoperatif ve postoperatif sekizinci haftada timpanogram, östaki tüp fonksiyon (ÖTF) testi yapıldı. Adenoid hacmi ile ameliyat öncesi ve sonrası kulak bulguları (otoskopi, timpanogram, ÖTF testi sonuçları) ki-kare testi ile istatistiksel olarak karşılaştırıldı. Sonuçlar ÖTF testi bulgularına göre, (i) basınç -300 mmH2O'dan O'a yükseldiyse iyileşme, (ii) O'a yakın iken negatifleştiyse bozulma ya da (iii) değişmediyse aynı kalma şeklinde değerlendirildi. Bulgular: On iki olguda efüzyonlu otitis media, 18 olguda kronik tonsillit vardı. İki olguda mite allerjisi saptandı. Adenoid hacmi ortalaması 2 ml bulundu. Adenoid hacmi küçük ( 1.9 ml) (n=16) olan 17 kulakta iyileşme gözlendi; 13 kulakta sonuçlar aynı iken, iki kulakta kötüleşme görüldü. Adenoid hacminin östaki tüp fonksiyonunu anlamlı derecede etkilemediği görüldü (p>0.05). Fakat ventilasyon tüpü uygulananlarda, sadece adenoidektomi veya tonsillektomi uygulananlara göre östaki tüp fonksiyonunun daha iyi olduğu saptandı (p<0.005).Sonuç: Çalışmada adenoid hacminin ÖTF'ye direkt etkisi gösterilemedi.
Abstract
Objectives: The effect of extrinsic compression of adenoid volume on the eustachian tube function (ETF) and the resulting tube obstruction were studied, Design and Methods: Twenty-five children (mean age 6.7 years, range 2-12 years) undergoing adenoidectomy alone or with concomitant procedures (tonsillectomy, ventilation tube insertion) were evaluated by tympanogram and ETF tests performed pre-operatively and in the eighth postoperative week. Adenoid volumes were compared with ear findings from tympanograms and ETF tests. The results were evaluated as follows depending on the intratympanic pressures obtained in the ETF tests: (i) increase from -300 mmH2O to 0 as improvement, (ii) decrease from 0 mmH2O as deterioration, and (iii) no change. Statistical analyses were made using the chi-square test. Results: Otitis media with effusion was detected in 12 patients, chronic tonsillitis in 18 patients, and mite allergy in two patients. Mean adenoid volume was 2 ml. In patients with small adenoid size (<1.1 ml) (n=2), improvement was achieved in three ears, while no .change was observed in one ear. Of those with middle adenoid size (n=7) (1.1 ml≤size≤1.9ml), improvement was seen in four ears, while 10 ears remained unchanged. In patients with large adenoid size (n=16) the results were as follows: improvement in 17 ears, similar in 13 ears, and deterioration in two ears. The volume of adenoid tissue did not show any significant effect on ETF (p>0.05). However, compared with adenoidectomy or tonsillectomy alone, significant improvement in ETF was found in patients undergoing adenoidectomy and ventilation tube insertion (p<0.005). Conclusion: A direct effect of adenoid volume on the eustachian tube function was not demonstrated.
