Özet
Amaç: Kronik otitis medianın intrakranyal komplikasyonlarını değerlendirmek.Hastalar ve Yöntemler: Kronik otitis medianın intrakranyal komplikasyonu tanısıyla takip ve tedavi edilen 34 hastanın kayıtları retrospektif olarak incelendi. Bulgular: En sık komplikasyonlar menenjit (%50), lateral sinüs tromboflebiti (LST) (%17) ve serebellar abse (%12) idi. Menenjitli olguların (n=24) %50'sinde, LST'li olguların (n=7) %57'sinde hem granülasyon dokusu hem kolestatom gözlendi. Menenjitli olguların %84'ünde, LSTli olguların tümünde dura açıktı. Serebellar veya temporal abseli olguların tümünde teğmen intakt bulundu.Sonuç: Menenjit ve LST'li hastalarda mastoid teğmenin yenik, duranın granülasyon dokusuyla kaplı olması, komplikasyon gelişiminde granülasyon dokusunun rolüne dikkat çekmiştir, intrakranyal abseli hastalarda komplikasyonun indirekt yolla geliştiği düşünüldü.
Abstract
Objectives: To evaluate intracranial complications of chronic otitis media.Patients and Methods: Thirty-four patients who underwent surgery for intracranial complications of chronic otitis media were retrospectively reviewed. Results: Meningitis (59%), lateral sinus thrombophlebitis (LST) (17%), and cerebellar abscess (12%) were the most common complications. Both granulation tissue and cholesteatoma were 50% and 57% in meningitis (n=24) and LST (n=7), respectively. The dura was open in 84% of patients with meningitis and in all patients with LST. All patients with cerebellar or temporal abscesses had intact dura. Conclusion: In cases with meningitis and LST, granulation tissue was the primary factor responsible for the development of complications, whereas in all cases of intracranial abscesses where the mastoid tegmen was intact, indirect routes may be possible.
