İmmünsuprese çocuk hastalarda Aspergillus cinsi küf mantarlarının neden olduğu invaziv fungal sinüzit

Özet
Amaç: Yoğun indüksiyon, konsolidasyon kemoterapisi almış veya kemik iliği nakli uygulanmış çocuk hastalarda gittikçe daha sık karşılaştığımız fungal sinüzitlerin tanı ve tedavisi değerlendirildi. Hastalar ve Yöntemler: Akut lenfoblastik lösemi (n=3), akut miyeloblastik lösemi (n=2), aplastik anemili (n=1) altı olguda (3 kız, 3 erkek; ort. yaş 12) Aspergillus türlerinin etken olduğu fungal sinüzit gelişti. Antifungal tedavi olarak lipozomal amfoterisin B verilirken, ayrıca dört hastaya itrakonazol, iki hastaya da itrakonazol+5-flusitozin uygulandı. Bulgular: Olgularda ateş (%100), yüz ve baş ağrısı (%83), yüz ve gözde ödemle birlikte duyarlık artışı (%50), burun tıkanıklığı (%33), yumuşak damak nekrozu sonucu gelişen damak perforasyonu (%33) ve oral kaviteye uzanan mantar topu (%33) gözlendi. Tüm olgularda, radyolojik olarak sinüsleri dolduran opasite ve hava-sıvı seviyeleri görüldü; lezyon yerleri ile boğaz ve-veya burun kültürlerinde Aspergillus türleri üretildi. Fungal sinüzit sırasında tüm olgularda nötropeni saptandı. (WBC<D.1x109-l). Tedavi ile dört olguda median 43 günde (20-96 gün) klinik ve radyolojik düzelme elde edildi. Kemik iliği nakli uygulanmış iki olgu sistemik fungal enfeksiyonla kaybedildi. Sonuç: İmmünsuprese hastalarda Aspergillus türlerinin etken olduğu sinüzitlerin tedavisi güç, uzun süreli ve çok pahalı olup mortaliteleri yüksektir.
Abstract
Objectives: We evaluated diagnosis and treatment of fungal sinusitis, which is on the incline among our pediatric patients receiving heavy induction, consolidation chemotherapy or bone marrow transplantation. Patients and Methods: Fungal sinusitis caused by Aspergillus species developed in six patients (3 girls, 3 boys; mean age 12 years) with acute lymphoblastic leukemia (n=3), acute myeloblastic leukemia (n=2), and aplastic anemia (n=1). All cases received liposomal amphotericin B; additionally itraconazole (n=4) and itraconazole and 5-flucytosine (n=2) were given. Results: Major complaints were fever (100%), headache and pain radiating to the face (50%), nasal obstruction (33%), soft palate perforation due to necrosis (33%), and fungus ball extending to the oral cavity (33%). Radiologically, opacity and air-water levels in the sinuses were detected in all cases. Aspergillus species were cultured from the lesions and samples from the throat and-or nose. At the time of the diagnosis of fungal sinusitis all patients were neutropenic (WBC<1x109-l) Clinical and radiologic improvement was achieved in four patients in a median of 43 days (20-96 days). Two patients died from systemic fungal infection following bone marrow transplantation. Conclusion: Treatment of sinusitis caused by Aspergillus species is difficult, time-consuming and very expensive in immunosuppresed patients, with high mortality rates.
