Özet
Amaç: Paranazal sinüs mukosellerinin klinik görünümleri ve tedavisi değerlendirildi. Hastalar ve Yöntemler: Paranazal sinüs mukoseli tanısı ile ameliyat edilen 13 hasta (10 erkek, 3 kadın; ort. yaş 40; dağılım 12-72) değerlendirildi. Paranazal sinüs mukoseli tanısı hastaların hepsinde bilgisayarlı tomografi (BT) ile kondu. Mukoselli hastalar klasik veya endoskopik tekniklerle ameliyat edildi. Hastalar ortalama altı ay ile 4 yıl arasında değişen sürelerle izlendi. Bulgular: Dört hastada frontal, iki hastada fronto-etmoid, üç hastada maksiller, iki hastada etmoid ve iki hastada sfenoid mukosel vardı. Beş hastada mukopiyosel bulundu. Sfenoid mukosel olan iki hastada ani görme kaybı vardı; bunların birinde cerrahi müdahaleden sonra görme kısmen iyileşti, diğer hastada düzelme olmadı. Sonuç: Çalışmamız, paranazal sinüs mukosellerinin en sık frontal ve fronto-etmoid sinüslerde bulunduğunu; sfenoid mukosellerde ani görme kaybı riskinin yüksek olduğunu ve paranazal sinüs mukosellerinin tedavisinde klasik teknikler yanı sıra endoskopik cerrahinin güvenilir olduğunu gösterdi.
Abstract
Objective: We evaluated the clinical features and treatment of paranasal sinus mucoceles. Patients and Methods: Thirteen consecutive patients who underwent surgery for paranasal sinus mucoceles were reviewed. There were 10 men and three women (mean age 40 years, range 12-72 years). Diagnoses were made by computed tomography. Surgery was performed by classic or endoscopic techniques. Patients were followed-up for a duration ranging from six months to four years. Results: The lesions were localized in the frontal (n=4), fronto-ethmoid (n=2), maxillary (n=3), ethmoid (n=2), and sphenoid (n=2) sinuses. Mucopyoceles were found in five patients. Sudden blindness developed in two patients with sphenoid mucopyoceles; following surgery, they had partial and no improvement, respectively. Conclusion: This study suggests that paranasal sinus mucoceles are most common in the frontal and fronto-ethmoid sinuses, that sphenoid mucoceles present increased risk of sudden blindness, and that the results of endoscopic treatment do not fall behind those of classic interventions.
