Otologic symptoms and findings in patients with a dehiscent or high jugular bulb

Özet

Amaç: Dehissan veya yüksek juguler bulb, seyrek rastlanan bulgulardandır; belirli semptomlarla kendilerini gösterebilirler. Kliniğimize herhangi bir nedenle başvuran ve çekilen temporal tomografisinde raslantısal olarak dehissan veya yüksek juguler bulb saptanan hastalarda otolojik semptom ve bulgular araştırıldı. Hastalar ve Yöntemler: Temporal kemik bilgisayarlı tomografik incelemeleri yapılan 156 hastanın 28'inde (16 erkek, 12 kadın; yaş ort. 39, dağılım 18-60) dehissan veya yüksek juguler bulb saptandı. Bu hastalarda dehissan veya yüksek juguler bulb'a eşlik eden otolojik semptom ve bulgular araştırıldı. Bulgular: Yirmi bir hastada (%13.5) yüksek juguler bulb, yedi hastada (%4.5) dehissan juguler bulb saptandı. Dehissan juguler bulb'lu hastaların beşinde (%71.4) tinnitus, birinde sensorinöral işitme kaybı gözlendi. Yüksek juguler bulb'lu hastaların ise sekizinde (%38.1) sensorinöral işitme kaybı, altısında (%28.6) tinnitus, dördünde (%19) baş dönmesi ve bir hastada Meniere hastalığı bulundu. Sonuç: Etyolojisi ortaya konamayan başdönmesi, tinnitus ve işitme kaybı ile başvuran bir hastada düşünülecek olası nedenler arasında dehissan veya yüksek juguler bulb varlığı da yer almalıdır.

Abstract

Objectives: A dehiscent or high jugular bulb is a rare finding, presenting with specific symptoms. We examined otologic symptoms and findings in patients in whom a dehiscent or high jugular bulb was documented as an incidental finding on computed tomography (CT) scans of the temporal bones. Patients and Methods: During evaluation of temporal bone CT scans of 156 patients, 28 patients were found to have a dehiscent or high jugular bulb. There were 16 males and 12 females, with a mean age of 39 years (range 18 to 60 years). Otologic symptoms and findings were investigated. Results: A dehiscent and a high jugular bulb were found in seven patients (4.5%) and 21 patients (13.5%), respectively. Of patients with a dehiscent jugular bulb, five patients presented with tinnitus (71.4%) and one with sensorineural hearing loss. The existence of a high jugular bulb was accompanied by dizziness (n=4, 19%), tinnitus (n=6, 28.6%), sensorineural hearing loss (n=8, 38.1%), and Meniere's disease (n=1). Conclusion: A high index of suspicion aids in the diagnosis of a dehiscent or high jugular bulb in a patient presenting with hearing loss, tinnitus, and dizziness of unknown etiology.
