Özet

Amaç: Baş-boyun defektlerinin rekonstrüksiyonunda pektoralis majör myokutan flebinin etkinliğini ve güvenirliğini değerlendirmek. Hastalar ve Yöntemler: Baş-boyun rezeksiyonu yapılan 33 hastada (2 kadın, 31 erkek; yaş ort. 60) gerçekleştirilen pektoralis majör myokutan flep rekonstrüksiyonu (n=34) retrospektif olarak incelendi. Rekonstrüksiyon yedi hastada sirkumferensiyel özofajeal defektler için, dokuz hastada mukozal örtme için ve 18 hastada dış deri kaplanması ve/veya yumuşak doku obliterasyonunda kullanıldı. Bulgular: Seride 11 komplikasyon (%32.3) gelişti. Beş olguda fistül, üç olguda parsiyel flep nekrozu, iki olguda yara enfeksiyonu, bir olguda serama ile karşılaşıldı. Hiçbir hastada tekniğe bağlanabilecek mortalite görülmedi. Sonuç: Pektoralis majör myokutan flebi ile rekonstrüksiyon, baş-boyun rekonstrüktif gereksinimlerinde güvenilir ve değerli bir seçimdir.

Abstract

Objectives: To evaluate the effectiveness and reliability of the pectoralis major myocutaneous flap in the reconstruction of head and neck defects. Patients and Methods: A retrospective review of 34 reconstructions with the pectoralis major myocutaneous flap was made in 33 patients (2 women, 31 men; mean age 60 years) undergoing head and neck resections. Reconstruction was made for repair of circumferential esophageal defects (n=7), for mucosal lining (n=9), and for external skin coverage and/or soft tissue obliteration of the dead space (n=18). Results: Flap-related complications were seen in 11 patients (32.3%), including partial flap necrosis (n=3), pharyngocutaneous fistula (n=5), wound infection (n=2) and seroma (n=1). No specific mortality attributable to the technique occurred. Conclusion: Reconstruction with the pectoralis major myocutaneous flap remains a valuable choice in head and neck reconstructions.
