İnfertilite: Umutsuzluk perspektifinden bir inceleme

Özet
Amaç: İnfertilite tıbbi, psikiyatrik, psikolojik ve sosyal yönleriyle psişik travmaya neden olan bir kriz durumudur. Üreme tekniklerindeki gelişmeler, hastalara bir yönüyle umut olurken, diğer yandan özellikle kadınlarda üzüntü, kontrol kaybı, damgalanma gibi durumlar da yaratmaktadır. Bu doğrultuda bu çalışmada infertilite tedavisi gören kadınlarda umutsuzluk ve umutsuzluğu etkileyebilecek benlik saygısı, çift uyumu, kişilik özellikleri ve sosyodemografik değişkenlerin incelenmesi amaçlanmıştır. Gereç ve Yöntem: Kesitsel ve tanımlayıcı nitelikteki bu araştırma Ege Üniversitesi Tıp Fakültesi Hastanesi Aile Planlaması ve Kısırlık Uygulama ve Araştırma Merkezinde 2009-2010 tarihleri arasında yapılmıştır. Çalışmanın örneklemini 141 kadın oluşturmuştur. Veriler, Tanıtıcı Bilgi Formu, Beck Umutsuzluk Ölçeği (BUÖ), Rosenberg Benlik Saygısı Ölçeği (RBSÖ), Çift Uyum Ölçeği (ÇUÖ) ve Mizaç ve Karakter Envanteri (MKE) kullanılarak toplanmıştır. Bulgular: Yaş ortalaması 32.21 (SD=4.73) olan infertil kadınların RBSÖden aldıkları puan ortalaması 5.13 (SD=3.52), BUÖden aldıkları puan ortalaması 7.52 (SD=3.82) olarak belirlenmiştir. Çalışma grubu- nu oluşturan kadınlarda benlik saygısı %37.3ünde düşük, %45.6sında orta, %17.9unda yüksektir. Düşük gelirli grupta BUÖden alınan puanlar anlamlı düzeyde daha düşüktür (F=12.35, p<0.01). Görücü usulü ile evlenen kadınların umutsuzluk düzeyleri daha yüksektir (F=3.63, p<0.05). Kadınlarda çift uyumu ile umutsuzluk arasındaki ilişki negatif yönde orta düzeydedir (r=-0.445, p<0.01). Çalışmaya katılan kadınlardan infertilite tedavi görme süresi en uzun olanlar BUÖden daha yüksek puan almışlardır (F=2.88, p<0.05). Kadınların RBSÖ ve BUÖden aldıkları puanlar arasında pozitif yönde anlamlı ilişki bulunmuştur (r=0.445, p<0.01). Regresyon analizi sonuçlarına göre sorumluluk, beceriklilik ve kendini unutma şeklindeki üç karakter özelliği kadınlarda umutsuzluğu yordayan değişkenler olarak saptanmıştır (F=16.36; p<0.01). Sonuç: Benlik saygısı, çift uyumu ve karakter özellikleri infertil kadınlarda umutsuzluğu etkileyen değişkenlerdir. İnfertilite tedavi sürecinde destekleyici psikososyal yaklaşımlara yer verilmesi yararlı olacaktır.
Abstract
Objectives: An Infertility medical, psychiatric, psychological, and social aspect of the psychic trauma is causing a crisis situation. Developments in reproduction techniques and patients with a direction of hope, especially in women dealing with sadness and loss of control, are creating situations such as stigma. In this study, women dealing with despair and hopeless- ness undergoing infertility treatment were examined for variables that may contribute to the symptoms, including self-esteem, dyadic adjustment, per- sonality traits, and socio-demographics. Methods: The cross-sectional and descriptive nature of this research was granted by Ege University Medical Faculty Hospital. Patient history between 2009-2010was obtained from the Family Planning and the Infertility Re- search Center. The study sample consisted of 141 women. Data was collect- ed using an Information Form, Beck Hopelessness Scale (BHS), Rosenberg Self-Esteem Scale (RSES), Dyadic Adjustment Scale (DAS), and the Tempera- ment and Character Inventory (TCI). Results: The mean age was 32.21 (SD=4.73) and infertile women received a mean RSES score of 5.13 (SD=3.52). BHS scores were taken from the average of 7.52 (SD=3.82). Self-esteem for the women in this study group was 37.3% low, 45.6% moderate, and 17.9% higher, respectively. In low-income groups, the BHS scores was significantly lower (F=12.35, p<0.01). The arranged mar- ried women felt higher levels of hopelessness (F=3.63, p<0.05). The relation- ship between dyadic adjustment in women with negative moderate levels of hopelessness received an r value of-0.445 (p<0.01). Infertility treatment in women participating in the study period scored a higher BHS than the long ones (F=2.88, p<0.05). Women between the RSES and receive their scores from BHS significant positive correlation was found (r=0.445, p<0.01). Ac- cording to the results of the regression analysis of responsibility, resourceful- ness and self- forget -shaped feature three characters in women has been identified as predictors of hopelessness (F=16:36, p<0.01). Conclusion: The self-esteem in infertile women couples harmony and character traits are variables affecting despair. In the process of infertility treatment will be useful to include supportive psychosocial approaches.
