H2 reseptör antagonisti ranitidin'in (Zantac) gastrointestinal tümörlü hastalarda T-supressor (T8) lenfosit, doğal öldürücü hücre aktivitesi ve tümör markerleri üzerine etkisi

Özet
H2 reseptör antagonistleri son yıllarda immunömodülatör bir ajan olarak tanımlanmaktadır. H2 reseptör antagonisti olan Zantac'ın (ranitidine) gast¬rointestinal tümörlü hastalarda immunömodülatör ajan olarak etkilerini araştırdık. Yirmişer kişiden oluşan iki gastrointestinal tümörlü hasta gru¬bundan birinci gruba sadece sistemik kemoterapi, ikincisine sistemik kemoterapi+H2 reseptör antagonisti uygulandı. Tüm hastalarda tedavi öncesi ve tedavi sonrası CEA, CA19-9, doğal öldürücü hücre aktivitesi ve T-supressor hücre düzeyleri ölçüldü. Sistemik kemoterapi uygulanan grupta doğal öldürücü hücre aktivitesi ve T-supressor hücre düzeylerinde anlamlı farklılık saptanmadı. Sistemik kemoterapi+H2 reseptör antagonisti uy¬gulanan grupta doğal öldürücü hücre aktivitesinde anlamlı artış ve T-supressor hücreleri düzeylerinde anlamlı düşüş tespit edildi (p<0.001). Sonuçta, gastrointestinal tümörlerde H2 reseptör antagonistlerinin immun cevabın düzenlenmesinde potansiyel bir etkiye sahip oldukları görüldü.
Abstract
H2 receptor antagonists have been described as immunömodülatör agents. We examined the immunomodulatory effect of ranitidine (Zantac) in patients with gastrointestinal tumors. Forty patients with gastrointestinal tumors were divided into two groups, each consisting of 20 patients. The groups were administered systemic chemotherapy alone and systemic chemotherapy+H2 receptor antagonist, respectively. In all patients CEA, CA19-9, natural killer cell activity and T-suppressor cell levels were evaluated before treatment and after treatment. In the systemic chemotherapy group no signifiant changes were found in the natural killer cell activity and T-suppressor cell levels, whereas in the systemic chemotherapy+H2 receptor antagonist administered group we noted an increased natural killer cell activity and decreased T-suppressor cell levels (p<0.001). Our findings confirm that H2 receptor an¬tagonists exert an immunomodulatory effect in patients with gastrointestinal tumors.
