Özet

Amaç: Bu çalışmada total larenjektomi sonrası trakeaözofajeal fistül ve Blom-Singer konuşma protezi uyguladığımız olgulara ait sonuçlar değerlendirilmiştir. Hastalar ve Yöntemler: 1996-1998 tarihleri arasında total larenjektomi yapılan toplam 14 hastadan, sekizine primer, altısına da sekonder ses restorasyonu uygulandı. Olguların hepsinde düşük basınçlı indwelling tip Blom-Singer konuşma protezi kullanıldı. Bulgular: Protez takılan 14 hastanın hepsinde uygulamayı takiben fonasyon sağlandı. Olgularımızın 12'si (%86) protezle konuşmayı devam ettirdi. Takılan protezler, iki olguda motivasyon yetersizliği yüzünden, bir olguda radyoterapi sonrası özofajeal striktür geliştiği için çıkarıldı. Özofajeal striktür gelişen olguda dilatasyon yapıldıktan sonra, konuşma protezi takılarak, tatmin edici bir konuşma sağlandı. Sonuç: Düşük basınçlı, indwelling tip konuşma protezinin total larenjektomi sonrası ses restorasyonunda, etkili ve güvenli bir araç olduğu düşünüldü.

Abstract

Objectives: We evaluated the results of tracheoesophageal puncture and Blom-Singer voice prosthesis in patients undergoing total larengectomy. Patients and Methods: Fourteen patients who underwent total larengectomy between 1996 and 1998 had primary (n=8) and secondary (n=6) prosthetic voice restoration using low-pressure indwelling Blom-Singer voice prosthesis. Results: Phonation was obtained in all patients following the placement of voice prosthesis. Of these, satisfactory speech rehabilitation was achieved in 12 patients (86%). Three patients required removal of prosthesis due to insufficient motivation (n=2) and radiotherapy-induced esophageal stricture (n=1). In the latter, following dilatation, voice prosthesis was reinserted and satisfactory voice was obtained. Conclusion: Our results suggest that the low pressure indwelling prosthesis is an effective and safe device for voice restoration following total larengectomy.
