Özet
Amaç: Oroantral fistüllü olgularda etyoloji, tedavi yöntemleri ve tedavi başarısını etkileyen faktörler değerlendirildi. Hastalar ve Yöntemler: Kronik oroantral ve oronazal fistül nedeniyle cerrahi tedavi gören 25 hastanın kayıtları retrospektif olarak incelendi. Bulgular: Fistüller, lokal mukoperiosteal flepler ile onarıldı. Etyolojik faktörler 11 olguda (%44) diş çekimi, beş olguda (%20) tümör, üç olguda (%12) osteomyelit, iki olguda (%8) Caldwell-Luc ameliyatı, iki olguda (%8) travma, bir olguda (%4) dentijinöz kist, bir olguda (%4) septum perforasyonu onarımı şeklinde saptandı. Fistüllerin üçü oroantronazal, biri oronazal ve 21'i oroantral fistül idi. İki hastada ikinci ameliyat sonrası başarıya ulaşıldı; diyabetli bir hasta ve kanser cerrahisi sonrası radyoterapi almış üç hastada cerrahi tedavide başarılı olunmadı. Bu hastalar protez ile tedavi edildi. Diğer hastalarda tek ameliyat sonrası başarı sağlandı.Sonuç: Çalışma, diş çekiminin, oroantral fistülün en sık etyolojik faktörü olduğunu, kronik oroantral fistüllü her hastada mutlaka tümör araştırılması ve ekarte edilmesi gerektiğini, sistemik hastalığı olan ve radyoterapi almış hastalarda sonucun başarısız olabileceğini, oroantral fistüllü hastalarda nadiren birden fazla cerrahi işlem gerekebileceğini ortaya koydu.
Abstract
Objectives: We evaluated the etiology, methods of treatment and the factors influencing success rates in patients with chronic oroantral fistulas. Patients and Methods: A retrospective analysis was made of 25 patients who underwent surgery for chronic oroantral and oronasal fistulas. Results; Fistulas were closed with local mucoperiosteal flaps. Etiologic factors were tooth extraction (n=11, 44%), tumor (n=5, 20%), osteomyelitis (n=3, 12%), Caldwell-Luc operation (n=2, 8%), trauma (n=2, 8%), dentigerous cyst (n=1,4%), and reconstruction of septal perforation (n=1, 4%). Fistulas were oroantro-nasal (rr=3), oronasal (n=1) and oroantral (n=21). Successful closure required two operations in two patients. Failure was encountered in patients with diabetes mellitus (n=1) and those receiving radiotherapy (n=3) after cancer surgery. They were treated with prosthesis. Single operation was sufficient for successful outcome in the remaining patients. Conclusion: The study showed that tooth extraction is the most frequent etiologic factor of oroantral fistulas, that neoplasms should always be investigated and excluded in chronic cases, that closure may not be successful in patients with systemic diseases and in those receiving radiotherapy, and that, although infrequent, more than one operation may be required for successful closure.
