Kliniğimizde uygulanan mainz II poş operasyonlarının değerlendirilmesi

Özet
Kasa invazif mesane tümörü tanısı almış 18 hastaya radikal sistektomi sonrasında Mainz II poş operasyonu uygulandı. Postoperatif ilk yedi günde 2 hastada (% 11) hipopotasemi, 2 hastada (% 11) hiponatremi ve 5 hastada (% 28) hipokalsemi gözlendi. Yedinci günden başlayarak oral beslenme, ek özel diyet ve günlük sodyum bikarbonat uygulamasına başlandı. Postoperatif üçüncü ayda 2 hastada metabolik asidoz gelişti. Üçüncü ayda elde edilen poş grafilerinde hiçbir hastada reflü gözlenmedi. Mainz II poş operasyonu sonrasında görülebi-len metabolik komplikasyonlar, ek özel diyet ve bikarbonat uygulanması ile önlenebilir. İyi uygulanmış bir ürointestinal anastomoz ve oluşturulan yüksek kapasiteli, düşük basınçlı rezervuar ise reflü sıklığını azaltabilir.
Abstract
A Mainz II type pouch operation was performed in 18 patients who underwent radical cystectomy for muscle invasive bladder cancer. Hypopotasemia in 2 patients (11 %), hyponatremia in 2 patients (11 %) and hypocalcemia in 5 patients (28 %) were observed within the postoperative first 7 days. Oral intake, additional special diet and bicarbonate prophylaxis were started on the seventh day. Metabolic acidosis developed on 2 patients on the third postoperative month. Pouchograms remained free of reflux in all patients. Metabolic complications of Mainz II pouch operations can be prevented by specific dietary cautions and bicarbonate prophylaxis. A properly performed ürointestinal anastomosis and formation of a high capacity low pressure reservoir will reduce reflux incidence.
