Metastatik nonseminomatöz germ hücreli tümörlerde kemoterapi sonrası cerrahi

Özet
Nonseminomatöz germ hücreli tümör nedeniyle kemoterapi alan ve tedavi sonrasında tümör belirleyicileri normale dönen, ancak kitlesi kaybolmayan 17 olguya retroperitoneal kitle eksizyonu yapıldı. Üç olguda teratom (% 17.6), 3 olguda nekroz (% 17.6) ve 11 olguda karsinom (% 64.8) saptandı. Yaşayan tümör hücresi bulunan olgulara ek 2 kür kemoterapi verildi. Ortalama 32.4 aylık (10 ile 63 ay arasında) izlem süresinde 15 olgu hastalık-sız yaşamaktadır. Bir olgu inkomplet rezidüel kitle eksizyonu ve daha sonra hastalğın yayılması nedeniyle ve 1 olgu beyin metastazı nedeniyle kaybedilmiştir. Kemoterapi sonrasında tümör belirleyicileri normale dönen fakat retroperitoneal kitlesi küçülmeyen veya tamamen kaybolmayan olgularda, tomografide görüntülenen ve/veya eksplorasyonda tanımlanan kitlelerin eksizyonu ile mükemmel sonuçlar elde edilebilmektedir.
Abstract
Surgical resections were performed 'in 17 patients with nonseminomatous germ cell tumors, whose tumor markers returned to normal levels after chemotherapy but retroperitoneal masses did not disappear. Teratoma in 3 patients (17.6 %) necrosis in 3 patients (17.6 %7 and residual carcinoma in 11 patients (64.8%) were found histologically. Additional two cycles of chemotherapy was given in patients with carcinoma. In a meanfollowup of 32.4 months (range; 10-63 months) 15 patients are alive ivith no evidence of disease. One patient died after imcomplete resection of the masses and disseminated disease and 1 patient died of brain metastases. Complete excision of radiologically determined residual masses and macroscopically suspicious neighboring nodes in patients with persistent retroperitoneal masses but normal tumor markers after chemotherapy has given excellent results in the follow-up.
