Renal ven ve vena kava inferiora uzanan renal hücreli karsinom: Sekiz hastadaki deneyim

Özet
Renal ven ve vena kava inferiora uzanan tümör trombüsü içeren renal hücreli karsinom manipülasyonu zor olan hastalıklardan birisidir. Bu çalışmada, inferior vena kava trombüsü varlığının cerrahi manipülasyonu ve prognoz üzerine etkisi incelenmiştir. Sekiz yıllık bir süre içinde böbrek tümörü tanısıyla nefrektomi yapılan 176 hastadan 8'inde (% 4.5) vena kava tümör trombüsü saptandı.Tümör trombüsü hastaların 3'ünde suprahepatik, 1'inde intrahepatik ve 4'ünde infrahepatik yerleşimliydi. Tümör ve trombus eksizyonu için 4 hastada abdominal insizyona sternotomi eklendi. Üç hastada ekstrakorporeal dolaşım ve hipotermi kullanıldı. Tümör trombüsünün en sık sağ böbrekte görüldüğü ve seviyesinin kitle boyutu ve prognozla anlamlı bir ilişki göstermediği belirlendi. Sekiz hastadan 3'ü kaybedildi. Halen izlemde olan 5 hasta ortalama 50 aylık izlem süresinde hastalıksızdır. Vena kavada tümör trombüsü bulunan renal hücreli karsinomlarda gerektiğinde ekstrakorporeal dolaşım ve hipoterminin de kullanılması ile etkin tedavi sağlanabilmekte ve yeterli sağkalım elde edilebilmektedir.
Abstract
Renal cell carcinomas with tumor thrombi involving renal vein and inferior vena cava pose a significant problem in treatment. In this study, the management and the effect on prognosis of the presence of inferior vena cava thrombus in renal cell carcinoma were investigated. Vena cava tumor thrombi were detected in 8 of the 176 patients (4.5 %) who underwent nephrectomy because of renal tumors in an eight-year period. The levels of tumor thrombi were suprahepatic in 3, intrahepatic in 1 and infrahepatic in 4 of the patients. For the resection of the tumors and thrombi a sternotomy was added to the abdominal incision in 4 patients. Extracorporeal circulation and hypothermia was used in 3 patients. Tumor thrombi were mostly encountered in right sided tumors and their levels remained independent from both the tumor size and prognosis. Three of the 8 patients died. The remaining 5 patients are alive without evidence of disease in a mean followup of 50 months. Renal tumors with vena caval tumor extention can be totally resected by using extracorporeal circulation and hypothermia when necessary yielding satisfactory survival rates.
