Metastatik testis tümörlerinde multikemoterapi sonrası kurtarma (Salvage) cerrahisi deneyimimiz

Özet
Amaç: Metastatik germ hücreli teslis tümörlerinde multipl kemoterapiye rağmen tümör belirleyicileri normale inmeyen hastalarda uygulanan kurtarma cerrahisi sonuçlarının incelenmesi. Yöntem: 1992 ve 2002 yılları arasında metastatik testis tümörü olan 8 hastaya kemoterapi sonrası yüksek tümör belirleyicileri ve metastatik kitle varlığı nedeniyle kurtarma cerrahisi (rezidüel kitle rezeksiyonu) uygulanmıştır. Hastaların orşiyektomi patolojilerinin hepsi embriyoner karsinom komponenti içeriyordu. Tüm hastalara sisplatin tabanlı kemoterapiler uygulandı. Bulgular: Kurtarma cerrahisi öncesi 7 hastada sadece alfa fetoprotein yüksek iken, bir hastada ek olarak beta human koriyonik gonadotropin hormonu da yüksekti. Altı hastada retroperitoneal, 2 hastada torakal metastatik kitleye rezeksiyon uygulandı. Bir hasta dışında tüm hastalara komplet rezeksiyon yapıldı. İnkornplet rezeksiyon yapılan hasta dahil 6 hastanın rezeksiyon patolojisinde canlı tümör hücresi vardı. Bu hastalardan biri cerrahi sonrasında izlemden çıktı. Kalan 5 hastaya verilen ek kemoterapi sonrasında hastaların ikisi öldü, biri hastalıksız olarak, diğer ikisi hastalıklı olarak izlenmektedir. Patolojisinde tümör olmayan 2 hasta hastalıksız olarak izlemdedir. Böylece 8 hastanın 3'ünde (%37.5) başarılı olunmuştur. Cerrahi sonrası ortanca izlem süresi 8.5 aydır (6-34). Sonuç: Kemoterapiye rağmen rezidüel kitlesi ve yüksek serum tümör belirleyici düzeyleri bulunan tetsis tümörlü seçilmiş hastalarda kemoterapi sonrası kur-tarma cerrahisi yaşam süresi üzerine olumlu etkisi olan bir tedavi şeklidir.
Abstract
Objective: To review the outcome of salvage surgery in patients with metastatic testicular germ cell tumors and with elevated tumor markers after multiple chemotherapies. Methods: Between 1992 and 2002 we performed post-chemotherapy salvage surgery (residual mass resection) on 8 patients with sustained elevated minor markers and metastatic mass despite treatment. An embryonal carcinoma component was present in orchiectomy specimens of all patients. All patients recieved cisplatin based chemotherapies. Results: Before salvage surgery all patients had elevated serum alpha fetoprotein levels and one patient had an elevated, beta human chorionic gonadotropin level as well. Six patients underwent retroperitoneal and 2 patients underwent thoracal mass resections. Complete resections were achieved in all but one patient. Viable tumor cells were present in the histopathologic specimens of six patients (including the patient with incomplete resection). One of these patients was lost to fol-low-up after resection. Postoperalively, 5 patients received further chemotherapy. Two of these 5 cases died, one was alive with no evidence of disease and the remaining two patients were alive with disease. Two patients with no tumor in their histopathologic specimens were alive with no evidence of disease. Thus, the overall success rate was 37.5 % (3/8). The median postoperative follow-up period was 8.5 months (6-34). Conclusion: Postchemotherapv salvage surgery does have a positive effect on survival, in a selected group of testis cancer patients with residual masses and elevated serum markers despite chemotherapy.
