Özet

Serum prostat spesifik antijen (PSA) duzeyi yuksek bulunan benign prostat hiperplazili olgularda PSA yukselmesine nedeh olabilecek histopatolojik kriterlerin arastmlmasi. Hastalar ve Yontem: Ocak 1997 ile Haziran 1998 arasinda agik prostatektomi uygulanan 167 olgu retrospektif olarak degerlendirildi. Serum PSA degerlerinde yukselme yapabilecek patpjojisi olan hastalar calisma dismda birakildi. Toplam 167 olgudan normal PSA degeri olan 28 (Grup I) ve yuksek PSA degeri olan 28 (Grup II) olmak Qzere toplam 56 hasta calisma kosullanna uygun bulundu. Olgulann prostat parafin bfoklanndan randomize olarak kesitler alinarak PSA igin immunhistokimyasal olarak boyandi. Histolojik olarak inflamasyon skoru, infarkt skoru, PSA boyanma oranlan saptandi. Bulgular: Olgulann yas ortalamasi grup I'de 66.60±i.85 yil,: grup ll'de 69.79±1.47 yil idi. Gruplar arasinda PSA, prostat volilmu ve PSA dansitesi degerleri istatiksel olarak farkli bulundu (p< 0.05). Histolojik degerlendirmede ise gruplar yalnizca kronik inflamasyon skoru acisindan farkli idi (p< 0.05). Sonug: Kronik prostatit ve prostat volumu benign prostat hiperplazili olgularda serum PSA artisina neden olan en onemli faktorlerdir.

Abstract

Histopathologic Factors That Probably Cause Increased Serum Prostate Specific Antigen Levels In Patients With Benign Prostatic Hyperplasia- Objective: To investigate histopathological criteria that could be responsible for increased prostate specific antigen (PSA) levels in benign prostatic hyperplasia patients. Patients and Methods: A total of 167 patients who had undergone open prostatectomies between January 1997 and June 1998 were evaluated, retrospectively. Patients who had overt lesions that increase PSA levels were excluded. Finally the study groups consisted of 28 patients with normal PSA levels (Group I) and 28 patients with high PSA levels (Group II) with a total of 56 out of 167. Random sections were obtained from paraffin, blocks of prostatic specimens and stained for PSA immunohistochemically. The inflammation score, infarct score and ratio of positive PSA staining were determined histologically. Results: The mean ages of the patients were 66.6 ± 1.85 and 69.79± 1.47 years in groups I and II, respectively. Significant differences between the two groups were noted with regard to PSA levels, prostate volumes and PSA density values (p< 0.05). Histological evaluation revealed significant difference between groups in only chronic inflammation scores (p< 0.05). Conclusion: Chronic protatitis and prostate volume are the most prominent factors that lead to increased PSA levels in patients with benign prostatic hyperplasia.
