Renal hücreli kanserlerin prognozunda anjiyogenezin yeri

Özet
Amaç: Renal hücreli kanserlerde anjiyogenezisin bir göstergesi olan mikrodamar sayısı (MDS) ile tümör büyüklüğü, histopatolojik tip, nükleer grade ve patolojik evre arasındaki ilişkinin incelenmesi ve anjiyogenezisin prognostik bir faktör olarak öneminin araştırılması. Yöntem: Klinik ve patolojik olarak böbrek kanseri tesbit edilmiş ve radikal nefrektomi uygulanmış 44 hasta çalışmaya alınmıştır. MDS, bu hastaların patoloji preparatlarında, immünhistokimyasal olarak fare CD 34 antikoru kullanılarak saptanmıştır. Sonuçlar hastaların klinik ve patolojik değişkenleri ile karşılaştırılmıştır. Bulgular: Hastaların 16'sı kadın, 28'i erkek ve yaş ortalaması 57.4 yıldır. Ortalama takip süresi 33.2 (3-39) aydır. Tümör büyüklüğü, histopatolojik tip ve nükleer grade gruplarının MDS ortalamaları arasında istatistiksel olarak fark bulunmazken, patolojik evre I-II ile III-IV'ün MDS ortalamaları istatistiksel olarak farklı saptanmıştır (p< 0.05). MDS ile patolojik evre pozitif (r= 0.3), hasta yaşam süreleri ise negatif (r= -0.34) korelasyon göstermiştir. Sonuç: Renal hücreli kanserlerde anjiyogenezisin bir göstergesi olarak MDS prognostik bir faktör olarak kullanılabilir, İlerlemiş ve metastatik kanserlerdeki yüksek anjiyogenez varlığı, bizi anjiyogenezisi hedefleyen yeni tedavi seçeneklerinin arayışına yönlendirebilir.
Abstract
Objective: To determine the relationship between microvessel count (MVC), as a marker of angiogenesis, and the tumor characteristics including tumor size, histopathologic pattern, nuclear grade and pathologic stage of renal cell carcinoma; and to evaluate the significance of angio genesis as a prognostic factor. Methods: Clinical and pathologic findings of 44 patients with renal cell carcinoma treated with radical nephrectomy were reviewed. MVC was evaluated inwumohistochemically using monoclonal mouse CD 34 antibody. The MVC findings were correlated with the patients' clinicopathologic variables. Results: Of the patients, 16 were females, 28 were males with a mean age of 57.4 years. The mean follow up was 33.2 (range; 3-39) months. There were no statistically significant differences between the mean MVC values of patient groups stratified accord-ing to tumor size, histopathologic pattern and nuclear grade. However, the difference between the mean MVC values in patho-logic stages 1-H and Ill-TV was statistically significant (p< 0.05). MVC values showed positive (r= 0.3) and negative (r= -0.34) correlations with the pathologic stages and the patient survival, respectively. Conclusion: Microvessel count as a mark-er of angiogenesis can be used as a prognostic factor in renal cell carcinoma. The presence of high angiogenesis in advanced and metastatic cancer states can lead us to think about new treatment models targeting angiogenesis.
