Nükleer matriks protein (NMP) 22 mesane tümörü teşhis ve takibinde sistoskopiye olan gereksinimi azaltabilir mi?

Özet
Amaç: Mesane tümörü izleminde altın standart olmasına rağmen invazif bir girişim olan sistoskopi ile nükleer matriks protein 22 (NMP22) tümör belirleyicisinin tanı ve izlemdeki yerinin karşılaştırılması. Yöntem: Toplam 78 hastadan oluşan çalışmada, 54 hastanın oluşturduğu Grup l polikliniğe hematim, sistizm veya prostatizm yakınmaları ile başvuran mesane tümörü olma riski yüksek hastaları içerdi. Grup 2 ise daha önce mesane tümörü nedeniyle tedavi edilip takip altındaki 24 hastayı içerdi. NMP 22 testi üreticinin önerileri izlenerek esas olarak ELISA yöntemi ile yapıldı. Sınır değeri olarak 15 U/mL kabul edildi. Bulgular: Grup l için NMP 22 duyarlılığı %82.3, özgüllüğü ise %50 olarak tespit edildi. Grup 2 için ise duyarlılık %100, özgüllük ise % 94.7 olarak belirlendi. Sonuç: NMP 22'nin mesane tümörü tanısında sistoskopinin yerini alacak kadar ideal bir tümör belirleyicisi olmadığı, ancak izlemde düşük riskli ve NMP 22 sonucu negatif olan olgularda kontrol sistoskopilerinin arasının açılabileceği, yüksek risk grubunda bulunan ve NMP 22 pozitif olan mesane tümörlü hastalarda ise tanının sistoskopi ile doğrulanması gerektiği görüşüne varıldı.
Abstract
Objective: To compare the gold standard but invasive cystoscopy and nuclear matrix protein (NMP) 22 in the diagnosis andfol-lowup of bladder tumors. Methods: A total of 78 patients were included into this study. Group 1 comprised 54 patients who presented to urology outpatient clinics with hematuria, cystism and prostatısın symptoms highly suggestive of bladder cancer. The remaining 24 patients who were infollowup with known history of bladder tumors were in Group 2. The NMP 22 test was ba-sicly done with the ELISA method according to the manufacturer's instructions. The cut off value was 15 U/mL. Results: The sensitivity and the specificity of the NMP 22 test were 82.3% and 50%, respectively in Group I. The corresponding figures in Group 2 were 100% and 94.7%, respectively. Conclusion: NMP 22 is not an ideal tumor marker to the extent that it may rep-lace cystoscopy in the diagnosis of bladder cancer. However, it can be useful to lengthen the interval between follow-up cystoscopies in patients with low-risk tumors. Patients in high risk groups and positive NMP 22 results should undergo cystoscopy.
