Özet

Amaç: Ramazan orucunun iskemik koroner olay insidans ında bir değişim yapmadığı gösterilmiş olsa da; oruç tutmanın akut miyokard infarktüsü (AMİ) sirkadiyen ritmine etkisi ile ilgili yeterli veri yoktur. Bu çalışma ile Türk halkında oruç tutmanın AMİ sirkadiyen ritmi üzerindeki etkisini araştırmayı amaçladık. Hastalar ve Metot: Eylül 2007-Ağustos 2010 yılları arasında dört ramazan ayı dönemini kapsayacak şekilde toplam 102 ST elevasyonlu AMİ hastası çalışmaya dahil edildi. Hastalar oruç tutup tutmamalarına göre iki gruba ayrıldı. İki grup demografik ve klinik özellikler ile AMİ sirkadiyen ritmi açısından karşılaştırıldı. AMİ sirkadiyen ritmi analizi için bir gün dört eşit saat dilimine bölündü ve 00:01-06:00, 06:01-12:00, 12:01-18:00 ve 18:01- 24:00 şeklinde sınıflandırıldı. Bulgular: Genel olarak bakıldığında AMİ başlangıç saati sirkadiyen ritim gösteriyordu ve hastalar sırasıyla 12:01- 18:00 ve 18:01-24:00 saat dilimlerinde yoğunlaşmıştı (p:0,03). Oruç tutmayan grupta AMİ başlangıç saati 12:01-18:00 saat diliminde pik yaparken (p<0,001); oruç tutan grupta hastaların büyük kısmı sırasıyla 18:01-24:00 ve 00:01-06:00 saat dilimlerinde yer alıyordu (p<0,001). Oruç tutan grupta hastaların yaklaşık % 60’nın göğüs ağrısı başlangıç zamanı iftar veya sahur yemeği sonras ındaki zaman dilimine denk geliyordu. Sonuç: Ramazan orucu beslenme ve uyku alışkanlıklar ında değişiklik yaparak AMİ sirkadiyen ritmini değiştirebilmektedir. AMI riski taşıyan oruç tutan kişilerde etkili beslenme ve uyku düzeni oluşturulması veya oruç tutmanın önerilmemesi AMİ sirkadiyen ritmin değişimi azaltabilir.

Abstract

Objectives: It has been shown that, Ramadan fasting did not change the incidence of ischemic coronary events. However, there is not sufficient evidence on the effect of Ramadan fasting on the circadian pattern of acute myocardial infarction (AMI). With this study; we aimed to show the effect of Ramadan fasting on the circadian variation of AMI in the Turkish population. Patients and Methods: Between September 2007 and August 2010, in four subsequent Ramadan months, a total of 102 AMI patients with ST elevation were enrolled into the study. Patients were divided into two groups according to the fasting habits. The groups were compared in terms of demographic and clinical characteristics, and the circadian pattern of the AMI. For the assessment of the circadian pattern of the AMI, a day was divided into four equal time periods and they were classified as 00:01-06:00, 06:01-12:00, 12:01- 18:00 and 18:01-24:00 hour periods, respectively. Results: Onset of the AMI showed significant circadian variation within all cohorts and most of the patients were accumulated in the 12:01-18:00 and 18:01-24:00 groups (p:0,03). Although, a rise in the events was observed in the non-fasting group in the afternoon (12:01-18:00, p<0.001), fasting patients tended to group in the 18:01-24:00 and 00:01-06:00 hour periods (p<0,001). Nearly 60% of the fasting patients experienced onset of the chest pain after the evening or night meals. Conclusion: Ramadan fasting could change the circadian pattern of AMI by altering feeding and sleeping habits. Either rearrangement of the feeding and sleeping patterns or suggesting not to fast to people who possesses high risk for an AMI could decrease the variation of the circadian pattern in the AMI.
