Metabolik sendromu olan ve olmayan obez hastaların antropometrik ölçümleri, yüksek sensitif CRP ve ürik asit düzeyleri açısından karşılaştırılması

Özet

Obezitenin morbidite ve mortaliteyi artırıcı etkisi bilinmektedir. Epidemiyolojik araştırmalarda metabolik sendromlu (MS) olgulardaki artış ile obezite epidemisi arasında yakın ilişki saptanmıştır. Bu çalışmanın amacı MS olan ve olmayan obez hastaların antropometrik ölçümleri, kardiyovasküler hastalık riskinde önemli olan yüksek sensitif CRP (hsCRP) ve ürik asit düzeyleri arasındaki ilişkinin değerlendirilmesidir. Materyal ve Metot: Polikliniğe başvuran 96 obez hasta kesitsel olarak değerlendirildi. Hastaların fizik muayene, antropometrik ölçümleri, karaciğer ve böbrek fonksiyon testleri, lipit parametreleri, ürik asit ve hsCRP düzeyleri değerlendirildi. Doksan-altı obez hastanın 58'i MS kriterlerini karşılarken, 38'i karşılamıyordu. Bulgular: Doksan-altı hastanın 77'si (%80) obez [Beden kitle indeksi (BKİ)≥30 kg/m²], 19'u (%20) ise morbid obezdi (BKİ≥40 kg/m²). Ürik asit düzeyi erkeklerde (p=0,00), hsCRP düzeyi ise kadınlarda daha yüksek bulundu (p=0,008). Metabolik sendromu olan obez ve morbid obez gruplar karşılaştırıldığında morbid obez grupta hsCRP düzeyleri istatistiksel olarak yüksek saptandı (p=0,036). Metabolik sendromu olmayan obez ve morbid obez gruplar karşılaştırıldığında morbid obez grupta yine hsCRP düzeylerinin daha yüksek olduğu görüldü (p=0,03). Sonuç: Bu çalışmada morbid obezlerde MS varlığından bağımsız hsCRP düzeyleri yüksek saptandı. Bu da hsCRP'nin daha yüksek saptandığı morbid obezitenin kardiyovasküler hastalık riskinin artışında önemli bir antite olduğunu göstermektedir.

Abstract

Morbidity and mortality increasing effect of obesity is known. In epidemiological investigations a close relation between the increment of patient with metabolic syndrome (MS) and obesity epidemic has been detected. The aim of this study is to evaluate the relationship between anthropometric measurements, high sensitive CRP (hsCRP) and uric acid levels of obese patients with and without MS, which are important at cardiovascular disease risk. Material-Methods: Ninety-six obese patients who applied to policlinic have been evaluated cross-sectionally. Physical examination, anthropometric measurements, liver and renal function tests, lipid parameters, uric acid and hsCRP levels of patients were evaluated. Fifty-eight of 96 obese patients fullfilled the criteria for MS while 38 did not. Results: Seventy-seven of 96 patients (%80) were obese [Body Mass Index (BMI)≥30 kg/m²] and 19 (%20) were morbid obese (BMI≥40 kg/m²). Uric acid levels in men (p=0.00) and hsCRP levels in women (p=0.008) were found higher. When the obese and morbid obese patients with MS were compared; hsCRP levels were statistically higher in the morbid obese group (p=0,036). High sensitive CRP levels were also higher in morbid obese group when the obese and morbid obese patients without MS were matched (p=0.03). Conclusion: In this study hsCRP levels were detected high at morbid obese patients independent from the presence of MS. This shows that morbid obesity where hsCRP is determined higher is an important entity for cardiovascular disease risk increment.
