İkinci Basamak Bir Hastanedeki Bruselloz Olgularının Değerlendirilmesi

Özet

Amaç: Brusella zoonotik bir hastalık olup ülkemizde endemik olarak görülmektedir. Bu çalışmada bruselloz olgularının klinik veri, tanı ve tedavi yöntemlerinin değerlendirilmesi amaçlandı. Materyal ve Metot: Hastanemizde 2011 Mayıs ile Kasım tarihleri arası bruselloz tanısı konulan 50 olgunun yaş, cinsiyet, klinik semptom ve bulguları, rutin laboratuvar sonuçları, Rose Bengal testi, Standart Tüp Aglütinasyon testi (STA), Coombs testi sonuçları ve tedavileri değerlendirildi. Bulgular: Hastaların demografik özellikleri incelendi- ğinde yaş ortalaması 38,2 yıl olup (16±78) %62’si er- kek, %38’i kadındı. En sık görülen klinik semptomlar eklem ağrısı (%72), miyalji (%54), halsizlik (%42), ter- leme (%42) ve iştahsızlıktı (%28). En sık başvurunun genel cerrahi (%36) bunu takiben dahiliye (%14), orto- pedi (%10), infeksiyon hastalıkları (%8), beyin cerrahisi (%10), fizik tedavi (%10), üroloji (%6) ve çocuk hasta- lıkları (%6) polikliniğine yapıldığı belirlendi. Hastaların 50’sinde Rose Bengal testi pozitif, 48’inde STA sonucu 1/160 ve üzerinde saptanırken, 2 olguda ise Coombs testi pozitif sonuç verdi. Tedavi protokolleri incelendi- ğinde %85 hastaya doksisiklin ve rifampisin kombi- nasyon tedavisi verildiği belirlendi. Sonuçlar: Spesifik brusella antikoru içermesine rağmen blokan antikorlar nedeniyle aglütinasyon vermeyen bruselloz şüpheli hasta serumlarının Coombs serumu ile çalışılması önemlidir. Değişik semptom ve bulgularla farklı polikliniklere baş- vurabilen, eklem ağrısı ve ateş öyküsü olan hastalarda bruselloz ayırıcı tanıda düşünülmesi gereken bir hasta- lıktır.

Abstract

Background: Brucella is a zoonotic disease and is endemic in our country. The aim of this study was to evaluate the clinical data, diagnosis and treatment methods of patients with brucellosis. Material and Methods: Age, gender, clinical symptoms, clinical findings, routine laboratory tests, Standard Tube Agglutination test (STA), Coombs test and the treatment methods were evaluated in fifty patients with brucellosis who were diagnosed with brucellosis in our hospital between May 2011 and November. Results: The demographic characteristics of the patients; mean age was 38.2 years (16±78), sixty-two percent of patients were male and thirty-eight percent were female. The most frequent symptoms were arthralgia (72%), myalgia (54%), fatigue (42%), sweating (42%) and anorexia (28%). Most of the applications of the patients were made for general surgery (36%), internal medicine (14%), orthopedics (10%), infectious diseases (8%), neurochirurgie (10%), physiotherapy (10%), urology (6%), and pediatrics (6%) clinics. Fifty patients showed a positive Rose Bengal test and STA test results were 1/160 or more in forty-eight patients. Tube agglutination test with coombs was positive in two patients. According to the treatment protocols, it was determined that eighty-five percent of the patients received a combination of doxycycline and rifampicin. Conclusions: In the suspicious patient serums with no agglutination despite their specific brucella antibody content as a result of blocking antibodies, investigation via Coombs reagent is particularly important. Brucellosis should be considered for the differential diagnosis of patients who are consulted by different clinics with different symptoms and findings and with a history of arthralgia and fever.
