Çocukluk Çağı Tüberkülozu Nedeniyle Takip Edilen Hastaların Değerlendirilmesi

Özet
Amaç: Tüberküloz halen ciddi mortalite ve morbidite sebebidir. Bu çalışmada, çocukluk çağı tüberkülozu nedeniyle takip edilen hastaların değerlendirilmesi amaçlanmıştır. Gereç ve Yöntem: Çalışmaya 109 hasta alındı. Hastaların kişisel ve ailesel demografik özellikleri, klinik, laboratuvar ve radyolojik incelemeler ile tedavileri retrospektif olarak kaydedildi. Bulgular: Hastaların %50,5’i erkek, %50’si kırsaldan geliyor ve %69,7’sinin geliri düşüktü. %58,3’ünün evinde sigara içiliyor, %23,9’unda aile içi ve %2,8’inde aile dışı temas saptandı. Hastaların en sık şikâyetleri öksürük ve ateş idi. En çok tüberküloz tutulum yeri akciğerlerdi. Hastaların %60,5’inde saflaştırılmış protein türevi (PPD) testi ve %44’ünde Bacillus Calmette-Guerin (BCG) skarı pozitifti. En sık görülen direkt grafi ve tomografi bulguları hiler ve mediastinal lenfadenopati idi. Hastaların sadece %12,9’unda en az bir yöntemle aside dirençli basil (ARB) pozitifliği ve %17’sinde kültürde üreme saptandı. Polymerase chain reaction (PCR) çalışılan 36 hastanın %33,3’ünde pozitif sonuç elde edildi. 12 hastada ise biyopsi ile tanı konulmuştu. Hastaların tedaviye uyumu %90 ve tam iyileşme %81,6 idi. Tartışma ve Sonuç: Tüberküloz eğitim ve sosyoekonomik düzeyi düşük, kalabalık ailelerde ciddi bir halk sağlığı problemidir. Aile içi ve dışı temas çocuklar için risk faktörüdür. Tanıda altın standart olan kültürde üretme yoluyla teşhis oranı düşüktür, bu yüzden alternatif tanı yöntemlerine ihtiyaç vardır.
Abstract
Introduction: Tuberculosis is still a major reason for mortality and morbidity. This study aims to evaluate followed up patients with childhood tuberculosis. Materials and Methods: 109 patients enrolled the study. Personal and familial demographics, laboratory, clinical and radiological examinations and the treatments of the patients were recorded, retrospectively. Results: 50.5% of the patients were male, 50% of the patient were from rural area and 69.7% of the patients had low income. The smoking rate at the house was 58.3%. There was intrafamiliar contact at 23.9% of the patients and extrafamiliar contact at 2.8%. The most frequent complaints of the patients were cough and fever. The most involvement area of the tuberculosis was lungs. Purified protein derivative (PPD) was positive at 60.5% of the patients, whereas Bacillus Calmette-Guerin (BCG) score was positive at 44% of the patients. The most seen direct graphy and tomography findings were hillar and mediastinal lymphadenopathy. 12.9% of the patients demonstrate Acid resistant bacilli (ARB) positivity with minimal method and reproduction detected at 17% of cultures. Positive results were obtained from 33,3% of the 36 patients, which Polymerase Chain Reaction (PCR) was performed. In 12 case, diagnosis was made by biopsy. The adaptation of the patients to the treatment was 90% and full recovery ratio was 81.6%. Discussion and Conclusion: Tuberculosis is a serious public health subject within crowded and low education and socioeconomic level families. Intrafamilial and extrafamilial contacts are risk factor for children. Diagnosis ratio with culture, which is a golden standard for diagnosis, is low and because of this reason, alternative diagnosis methods are needed.
