Özet

Bu çalışmada abdominal defektlerin kapatılmasında pediküllü seromüsküler jejunal otogreftin kullanılması amaçlanmıştır. Çalışmada toplam yedi adet sağlıklı ve erişkin köpek kullanıldı. Genel anestezi altında, median laparatomi ile karın boşluğuna girildi. Karın duvarında deneysel bir defekt (7x14 cm) oluşturuldu ve aynı büyüklükteki otojen, pediküllü jejunal segment ile onarıldı. İmplantasyondan önce, jejunal segment longitudinal bir ensizyonla açıldı ve bu segmentin mukozal katı bir bisturi ile kazındı. Sonra seromüsküler tabakası karın defekti bölgesine çekildi ve basit ayrı dikişlerle karın duvarına tutturuldu. Postoperatif altı ay süreyle takip edilen olgularda önemli bir komplikasyonla karşılaşılmadı. Yapılan histopatolojik muayenelerde jejunal otogreftin bölgeye mükemmel bir uyum sağladığı görüldü. Sonuç olarak fıtık deliklerinin kapatılmasında organizmaya uyum problemi yaratmayan ve sentetik materyallere göre kolay bulunan ve ucuz olan bu yöntemin kullanılabileceği kanısına varılmıştır.

Abstract

In this study, it was intended to repair experimentally created abdominal wall defects with the pedicled Seromuscular layers of the jejunum. For this purpose, seven healthy native .adult dogs were used. Under general anesthesia, the abdominal cavity was entered through mid-line incision. A defect (7x14 cm) was created experimentally on the abdominal wall and was repaired with the same size of autogen jejunal segement with mesenterial attachment. Prior to implantation, the jejunal segment was longitudinally sectioned and its mucosal layer was removed with blunt dissection. Then, it was transferred to the defect side and sutured to the abdominal wall with simple interrupted pattern. During six months follow-up period, no complication was observed and histopathological examination revealed that jejunal graft was well-tolarated with no evidence of tissue reaction. It is concluded that jejunal Seromuscular layer should be preferred to synthetic materials for the repair of hernial defects as it is easily accessible and also impose no harm effects on grafted region.
