Özet
Erkek 6 yaşlı bir kedi kusma ve konstipasyon şikayetleri ile kliniğe getirildi. Anemnezde konstipasyonun uzun süredir devam ettiği ve ağrılı defelcasyon yaptığı bildirildi. İnspeksiyonda kedinin dehidre, tüylerinin mat ve karışık olduğu gözlendi. Abdominal palpasyonda kolonlarda katı dışkı kitleleri ve gaz varlığı tespit edildi. Direkt radyografide biraz genişlemiş olan kolonların dışkı ve gaz ile dolu olduğu belirlendi. Kolonik düz kasların motilitesini henüz yitirmediği düşünülerek medikal sağaltım denendi. Parenteral sıvı ve antibiyotik uygulamasını takiben ılık sabunlu su lavmanı uygulandı. Klinik sağaltımı tamamlandıktan sonra Prescrption Diet Feline w/d maması kullanılmaya başlandı. Kolonik motiliteyi uyarmak için cisapride ve dışkıyı yumuşatmak için lactulose oral yolla uygulandı. Sağaltıma rağmen şikayetlerin artarak devam etmesi ve ilerleyen dönemlerde megakolonun tespiti sonucu operatif sağaltıma karar verildi. Proksimal kolonda aşırı bir genişleme olmadığından subtotal kolektomi yöntemi tercih edildi. Postoperatif dönemde parenteral sıvı ve antibiyotik sağaltımı uygulandı. Hasta iki yıl boyunca intestinal kuru mama ile ve son bir yıldır senior kuru mama ile beslendi. Bu dönemde defekasyonla ilgili herhangi bir probleme rastlanılmadı. Çalışmamızın da ortaya koyduğu gibi kronik olgularda ve diğer çalışmalarda bildirildiği üzere idiopatik mega-kolonda en radikal çözümün total ya da subtotal kolektomi olduğu saptanmıştır.
Abstract
A six-year-old male cat was brought to the clinic with chronic constipation and tenesmus with intermittant periods of vomiting. During inspection we observed that the cat was dehidrated and had a poor haircoat. A large amount of hard fecal metarial and gas was palpable in the colon. Direct radiographs showed the colons were larger and filled with feces and gas. We thought that the colonic smooth muscle was still functioning, so we decided to follow cause of medical therapy. Paranteral fluids and antibiotics were administered and given enemes using warm soapy water. As a follow up we recommended Prescription Diet Feline w/d. During the therapy cisapride was administered to stimulate colonic motility and lactulose administered to soften the feces. Despite the medical treatment, the illness progressed and megacolon developed, so we elected for surgical treatment. We prefered a subtotal colectomy because the proximal colon had not dilated much. Post operative treatment comprised parenteral fluid therapy and antibiotics. For a period of two years he used intestinal diets and later senior diets. There was no problem regarding his defecation until this time. Based on this particular case study it would appear that in cases related to chronic or idiopathic megacolons, a total or subtotal colectomy may be required.
