Düşük riskli yüzeyel mesane kanserlerinde operasyondan hemen sonra uygulanan tek doz mitomisin-C instilasyonunun kısa dönem rekürrensler (Tekrarlayan tümör) üzerine etkisi

Özet
Bu çalışmada düşük riskli yüzeyel mesane kanserlerinde tek doz ve operasyondan hemen sonra intrakaviter mitomisin-C uygulamasının kısa dönem rekürrensler (tekrarlayan tümör) üzerine etkisi araştırılmıştır. Prospektif, randomize bir çalışmayla düşük riskli yüzeyel mesane kanseri tanısı alan 40 hasta değerlendirmeye alındı. 3cm'nin altında tek tümörü olan, primer ve/veya son bir yıldır nüksü olmadan yüzeyel mesane kanseri saptanan hastalar çalışma grubuna dahil edildi. Kasa yayılmış olan, G3 tümör ve karsinoma-in-situ saptanan hastalar çalışma dışı bırakıldı. Randomizasyondan önce hastalara komplet TUR uygulandı. Hastalar iki gruba ayrılarak, bir grup sadece : takip edilirken, diğer gruba TUR sonrası hemen 40 mg intrakaviter tek doz mitomisin-C uygulandı. Hastalar 24 ay süreyle takip edildi. 24 aylık takipte kontrol grubunda 6 (%30) nüks saptanırken, mitomisin-C grubunda 3 (%15) nüks saptandı (p=0.45). Kontrol grubunda tekrarlayan tümöre (nükse) kadar geçen süre 6.5±3.5 ay olarak hesaplanırken, bu süre mitomisin-C grubunda 15.0±5.2 ay olmuştur (p=0.049). Düşük riskli yüzeyel mesane kanserlerinde operasyondan hemen sonra tek doz mitomisin-C uygulaması tekrarlayan tümör (nüks) oranlarını değiştirmemekte, tekrarlayan tümöre kadar geçen süreyi uzatmaktadır.
Abstract
The effectiveness of a single dose mitomycin-C instillation immediately after TUR on short-term recurrences in patients with low-risk superficial bladder tumors has been investigated in this study. A total number of 40 patients with, the diagnosis of low-risk superficial bladder cancer have been evaluated in a prospective, randomized trial. Patients with primary, single tumors 3cm or less and/or recurrent tumors which were recurrence-free in the previous year were enrolled in the study. Patients with muscular invasion, G3 tumors or carcinoma-in-situ were excluded from the study. All of the patient had a complete TUR before randomization and were divided into 2 groups: No further treatment or intracavitary single dose 40mg mitomycin-C. The follow-up period was 2 years. After 24 months of follow-up, 6 (30%) recurrences were encountered in the control group compared to 3 (15%) recurrences in the mitomycin-C group (p=0.45). The mean time to recurrence was 6.5±3.5 months in the control group where this .number was 15.0±5.2 months in mitomycin-C group (p=0.049). Our analysis confirms the positive effect of early single dose mitomycin-C instillation in patients with low-risk superficial bladder cancer with a longer period of time to recurrence and it does not seem to change the overall recurrence rate.
