Mikroskobik hematürinin tanısal değeri çocuk ve erişkin hastalarda aynı mıdır?

Özet
Çalışmamızın amacı, özellikle çocuk hastalarda mikroskobik hematürinin neden olacağı gereksiz, pahalı ve hasta için zor olabilecek tetkiklerin mümkün olan en asgari seviyede tutulabilmesi amacıyla, mikroskobik hematüri varlığının çocuk ve erişkindeki klinik önemi arasındaki farkı araştırmaktır. Bu retrospektif çalışmada üroloji polikliniğine çeşitli yakınmalarla başvuran ve mikroskobik hematüri saptanan 2314 hastanın verileri analiz edildi. Hastalar, ortalama yaşı 53,2±16,1 (17-85 arasında) olan 16 yaş üstü 2185 hasta erişkin grubu, ortalama yaşı 11.4±3.2 olan (1-16 arası) 16 yaş altı 129 hasta çocuk grubu olarak ayrıldılar. Hastaların detaylı anamnezi, fizik muayene, tam idrar tetkikleri ile radyolojik ve ürolojik değerlendirmeleri değerlendirmeye dahil edildi.Yapılan muayene ve tetkikler incelendi ve bulgular idrar mikroskobisindeki eritrosit sayısı esas alınarak değerlendirildi. Çocuk ve erişkin hastalarda idrar incelemesindeki eritrosit miktarının artmasıyla patolojik bir tanıya ulaşma oranı arttığı tespit edildi (sırasıyla p=0.000, p=0.000). Hematüri miktarı arttıkça erişkinlerde intravenöz piyelografi, ultrasonografi ve bilgisayarlı tomografide patolojik bulgu saptama oranının anlamlı olarak farklı olduğu tespit edildi sırasıyla p=0.000, p=0.006 ve p=0.004). çocuk hastalarda ise intravenöz piyelografi ve ultrasonografide patolojik tanı saptama oranı anlamlı olarak artarken bilgisayarlı tomografide aynı fark saptanmadı (sırasıyla p=0.025, p=0.039, p=0.713). Çocuklarda sistoskopi bulguları ile tanılar arasında ilişki saptanamazken (p=0.134), erişkinlerde anlamlı ilişki mevcuttu (p=0.001). Her iki yaş grubunda da hematüri miktarı arttıkça patolojik tanıya ulaşma oranları arasında anlamlı fark olduğu saptanmıştır (erişkin ve çocuklar için p=0.000). Asemptomatik çocuk hastalarda idrar tetkikinde 10 /HPF’den az eritrosit varlığında erişkinlerden farklı olarak ileri tetkik yöntemlerinden kaçınılmalıdır.
Abstract
Introduction: Microscopic hematuria should be evaluated because of early diagnosis and treatment of life threatening diseases. However, the evaluation of hematuria should have low cost and easy for patients. Several studies have been made to find a cut-off value for asymptomatic microscopic hematuria in children. The aim of this study was to investigate the difference of the meaning of hematuria between adults and children. Materials and Methods: In this retrospective study we evaluated 2314 patient who admitted to urology clinic and had microscopic hematuria in urinanalysis. 2185 patients older than 16 years old (17-85) were the adult group and 129 patient younger than 16 years old (1-16) included children group. All patients were evaluated with a detailed history, physical examination, urine analysis and radiological and urological examination. Children having the diagnosis of glomerular disease and the patological findings of glomerular disease as proteinuria, hipertension and edema were excluded from the study. The groups were made according to the degree of hematuria, the degree of pyuria, patological findings of intravenous urography, ultrasound, computerized tomography, cystoscopy, and also according to diagnosis, complaint, physical examination and age. All findings were compared between the two groups. Results: The most common problem among the two groups was urinary tract infection (adults: 29.7%, children: 37.9%). Both in children and in adults the rate of diagnosis raised as the hematuria raised up (p=0.000 and p=0.000 respectively). Also in both adult group and children group the pathological findings of IVP and USG significantly raised up as the hematuria rose up (p=0.000, p=0.006, for adults respectively; p=0.025, p=0.039, for children respectively). The same significant was also true for pathological findings of CT for adult group but not for children group (p=0.004, p=0.134, respectively). In children group there was no correlation between the cystoscopy findings and diagnosis (p=0.134), but there was statistically significant correlation between cystoscopy findings and diagnosis in the adult group (p=0.001). Conclusion: Further investigations should be avoided for the patients having asymptomatic microscopi hematuria that is lower than 10 erytrocytes/HPF.
