Selim prostat hiperplazili hastalarda selektif alfa-1 reseptör blokerlerinin serum total ve serbest prostat spesifik antijen (PSA) düzeyi ile serbest / total PSA oranı üzerine etkileri

Özet
Bu çalışmanın amacı, selim prostat hiperplazili (BPH) hastalarda doksazosin, terazosin, alfuzosin ve tamsulosinin serum total, serbest ve serbest/total prostat spesifik antijen (PSA) oranlarına etkilerini incelemektir. Bu prospektif çalışmaya, semptomatik BPH nedeniyle doksazosin 4 mg/gün (n: 25), terazosin 5 mg/gün (n: 26), alfuzosin 3x2.5 mg/gün (n: 26), tamsulosin 0.4 mg/gün (n: 20) alan ya da izlem (n: 17) protokolüne alınan 114 hasta dahil edildi. Bu beş gruptaki hastada, tedavi öncesi ve tedavi sırasında serum total PSA, serbest PSA ve serbest/total PSA oranları karşılaştırıldı. Ayrıca her grupta tedavi öncesi ve tedavi sırasında serbest/total PSA oranı $\leq$ %20 olan hastaların sayıları karşılaştırıldı. Bütün gruplarda tedavi sırasındaki total ve serbest PSA değerleri tedavi öncesine göre düşük olmasına rağmen istatistiksel olarak anlamlı değildi. İstatistiksel olarak anlamlı olmasa da, total ve serbest PSA'daki en fazla düşüş terazosin grubunda gözlendi (sırasıyla %16 ve %23.5). Tedavi öncesi ve tedavi sırasındaki serbest/total PSA oranları arasında istatistiksel olarak anlamlı bir fark izlenmedi. Gruplardaki tedavi öncesi ve tedavi sırasındaki serbest/total PSA oranı $\leq$ %20 olan hasta sayıları arasında fark gözlenmedi. Selektif alfa-1 reseptör blokerleri serum total, serbest ve serbest/total PSA oranlarını değiştirmemektedir. Çalışmamız, doksazosin, terazosin, alfuzosin ve tamsulosin kullanan BPH' lı hastalarda, rutinin dışında serum total ve serbest PSA ölçümlerine gerek olmadığını göstermektedir.
Abstract
The aim of this study is to determine the effect of doxazosin, terazosin, alfuzosin and tamsulosin on serum total and serum free prostate specific antigen (PSA) levels and the ratio of free to total PSA in patients with benign prostatic hyperplasia (BPH). In this prospective study, we included 114 men with symptomatic BPH who had been receiving 4 mg/day of doxazosin (n: 25), 5 mg/day of terazosin (n: 26), 3x2.5 mg/day of alfuzosin (n: 26), 0.4 mg/ day of tamsulosin (n: 20) or no therapy (watchful waiting, n: 17). Serum total, free, and free/total PSA levels were compared from pre-treatment to post-treatment among the five groups. We also compared the number of patients with PSA percentage of $\leq$ 20 during the therapy and before the treatment among the groups. In all groups, although the mean post-treatment total and free serum PSA levels were lower than pretreatment levels, the difference was not statistically significant. The terazosin group had the highest decreasing in post-treatment serum total and free PSA levels ( 16% and 23.5%, respectively), but it was not statistically different between the groups. However, the mean percent free PSA was not significantly different in the groups compared before treatment and during the treatment. The number of patients with percent PSA $\leq$ 20 was not significantly different before the treatment and post-treatment among the groups. Selective alpha-1 receptor blockers do not appear to alter serum total, free, and percent free PSA levels. Our study demonstrates that serial measurement of total and free PSA is not necessary in BPH patients treated with doxazosin, terazosin, alfuzosin and tamsulosin.
