Özet

Anatomik üretral hasarlarda cerrahi onarımın amacı obstrükte olmayan, kontinan bir neoüretra oluşturmaktır. İlk defa Heinrich Martius tarafından tanımlanan labial yağ flebi de üretral hasar onarımlarında giderek daha sıklıkla kullanılmaktadır. Bu çalışmada amacımız, anatomik üretral hasarların tedavisinde Martius labial yağ flebinin ek işlem olarak sonuçlarının değerlendirilmesidir. Bu çalışmada Aralık 1999-Haziran 2002 tarihleri arasında çeşitli nedenlerle üretral onarım yapılan kişilerde Martius labial yağ flebi destek olarak kullanılmış ve sonuçlar değerlendirilmiştir. Çalışma grubu 4'ü uzun dönem kateterizasyona bağlı üretral hasarlı, 2'si ise iyatrojenik nedenlere bağlı üretrovajinal fistüllü toplam 6 hastadan oluşmaktadır. Bütün hastalara Martius labial yağ flebi, üriner sistemle ilgili diğer rekonstruktif operasyonlara ek olarak gerçekleştirilmiştir. Takip süreleri 2-32 ay arasındadır. Hastalarda komplikasyon görülmemiş olup, tüm hastalar takiplerinde kontinandırlar. Sonuç olarak, Martius labial yağ flebi uygulaması anatomik üretral hasarların tedavisinde basan oranım arttırmaktadır ve bütün vakalarda kullanımını önermekteyiz.

Abstract

Introduction: The goals of surgical correction of the anatomical urethral defects are to create a continent neourethra which permits the unobstructed passage of urine. The Marti us labial fat flap that was first described by Heinrich Martius is getting more widely used at the repairing of urethral defects. In this study our purpose is to determine the results of Martius labial fat flap interposition as an additional procedure for the anatomical urethral defects. Materials and Methods: In this study, we evaluated the results of patients whom we were performed Martius labial fat flap interposition as an additional procedure with urethral reconstruction. Our working group has become from 6 patients whose, 4 have urethral defects because of long term catheterization, and 2 have urethrovaginal fistulas because of iatrogenic causes. Results: For all patients, Martius labial fat flap interposition was performed as an additional procedure with reconstructive operations of lower urinary tract according to original technique. Follow up ranged from 2 to 32 months. There were no complications and all the patients were continent on the follow up. Conclusion: As a result, Martius labial fat flap interposition improves the success rate of anatomical urethral defects and we recommend it for all cases.
