Yüzeyel ürotelyal hücreli mesane kanserlerinin takibinde p53 ve Ki-67 overekspresyonlarının yeri

Özet
1998 World Health Organization (WHO)/Interıiational Society of Urologic Pathologists (ISUP) Consenus sınıflamasına göre yeniden derecelendirilen "Evre Ta, G1-2" yüzeyel ürotelyal hücreli mesane kanserlerinin prognozuyla p53 ve Ki-67 overekspresyonlarının korelasyonunu araştırdık. Ocak 1995 ile Aralık 1999 tarihleri arasında primer yüzeyel ürotelyal hücreli mesane kanseri (evre pTa,G1-2) tanısı almış 48 hasta retrospektif çalışmamıza dahil edilmiştir. Vakalar 1998 WHO/ISUP Consensus sınıflamasına göre yeniden derecelendirildi. Buna göre hastaların alt gruplara göre dağılımı: 1-Ürotelyal papillom (ÜP): 7 (%14,6), 2-Düşük malin potansiyelli papiller ürotelyal neoplazi (DMP): 16 (%33,3), 3-Düşük grade papiller ürotelyal karsinom (DG): 14 (%29,2), 4-Yüksek grade papiller ürotelyal karsinom (YG): 11 (%22,9) şeklinde oluştu. Toplam 17 vakada görülen nüksün 1'i (%5,8) DMP, 8'i (%47,1) DG ve 8'i (%47,1) YG gruptadır. p53 ve K-67i'ye ait immünreaktivite indeksi (p53i, Ki-67i) ortalama değerleri sırasıyla ÜP'de %2,5 ve %3,8; DMP'de %6,6 ve %8; DG'de %19,9 ve %19,7; YG'de %35,1 ve %30,1 olarak bulundu. Nüks grubunda p53i ve Ki-67i ortalamaları sırasıyla %27 ve %23; nüks görülmeyen grupta ise %10 ve %11 bulunmuştur. p53i eşik değeri %22 alındığında nüks grubunda vakaların 13'ü pozitif, 4'ü ise negatiftir. Ki-67i eşik değeri %10 alındığında ise nüks grubunda vakaların 15'i pozitif, 3'ü negatiftir. İmmünohistokimyasal olarak belirlenen p53 ve Ki-67 immünreaktiviteleri nüks riskini belirlemede, multisentrisite ve tümör büyüklüğüne göre daha başarılı bulunmuştur. p53 ve Ki-67 indeksinin rutinde kullanılan prognostik faktörlere eklenmesi, hastalarda uygun ve etkili tedavi seçimi ile izlem sürecinin sıklığı, yöntemi ve tutumunun belirlenmesinde klinik yarar sağlayacağı kanısındayız.
Abstract
We determined the correlation between recurrence and overexpressions of p53 and Ki-67 in the prognosis of stage pTa, G1-2 superficial urothelial cell bladder cancers regraded according to the classification of 1998 WHO/ISUP Consensus. Forth eight patient who were diagnosed to have primary superficial urothelial cell bladder cancer (stage pTa,G1-2 between January 1995 and December 1999 were examined retrospectively. Subgroups of patients were as follows: 1-Urothelial papilloma (UP): 7 (14,6%), 2-Papillary Urothelial Neoplasm of Low Malignant Potential (LMP): 16 (33.3%), 3-Papillary Urothelial Carcinoma, Low grade (LG): 14 (29.2%), 4-Papillary Urothelial Carcinoma, High grade (HG): 11 (22.9%). Mean percentage values of p53 and Ki-67 (immunreactivity index) were found as 2,5 and 3,8% in UP; 6,6 and 8% in LMP; 19,9 and 19,7% in LG; 35,1 and 30,1 in HG respectively. If threshold percentage value of p53 were taken as 22%, 13 cases were positive and 4 cases were negative in recurrent group. When threshold percentage value of Ki-67 was taken as 10%, 15 cases were positive and 2 cases were negative in recurrent group. When tumor markers such as p53 and Ki-67 were together with other criteria, not even by their own, we concluded that superficial bladder tumors, which have high recurrency rate and progression risk, can be differentiated from others and so that proper cure and observation frequency can be planned according to this.
